Licensing Specialist reviewed the Landlord Agreement between the Provider and the home owners. However, the Provider stated that she does not have a current Liability Insurance policy. The one on file expired in April.
Licensing Specialist was not able to observe a current liability insurance policy. The one on file expired in April of 2025 and this was noted and requested during a previous inspection on February 27, 2025. Provider stated that a check payment was mailed out yesterday, but does not have evidence of such.
Provider informed Licensing Specialist that 2 documented residents no longer reside in the family child care home. Provider also informed Licensing Specialist that she is no longer residing in the home.
Provider stated that she has been staying with her ex-husband and her 10 year-old son in Germantown since receiving the Compliance Agreement. She leaves the family child care in the evenings and returns in the morning. Provider stated that all of her clothes and toiletries are at the ex-husband's home in Germantown. Licensing Specialist was not notified of this change. Started staying with ex-husband right after we served the emergency suspension.
Licensing Specialist did not observe a registration posted in the family child care.
Licensing Specialist did not observe daily attendance records for any of the children enrolled in care. Provider stated that she hasn't taken attendance since 2024 at her previous location.
Licensing Specialist did not observe a record of fire/emergency drills. Provider stated that she hasn't recorded any drills since 2024 at her previous location.
Licensing Specialist observed emergency forms without current information for at least (6) children enrolled in care. The emergency forms had an inaccurate enrollment date. One form was missing an enrollment date altogether.
Licensing Specialist observed (1) children's file missing part 2 of the health assessment and (1) child missing a lead test.
