On May 12, Director stated that she does not have access to New Hire records and cannot provide details about an employee's first day in the classroom. Two Employees told LS. Manager that they had started working in the program prior to being cleared.
LS observed one Staff member alone supervising 14 children between the ages of 3 and 5 year old in room 8, and one Staff member alone supervising 15 children between the ages of 3 and 5 years old in room 3. LS observed that there were 20 Children in a room with a Capacity of 19.
LS observed a soda can labeled "pinhole camera" on a window ledge about 7 feet from the floor in the upstairs children's bathroom. The pinhole camera had been placed there by a parent with Management's permission, but without the permission of the parents of children who use that bathroom.
LS observed one Staff member alone supervising 14 children between the ages of 3 and 5 year old in room 8, and one Staff member alone supervising 15 children between the ages of 3 and 5 years old in room 3 on May 11. LS Manager observed 2 Aides supervising a classroom of 14 children between the ages of 3 and 5 in room 9 on May 12.
LS observed that all children were being supervised by qualified Staff in all classrooms on May 11. LS Manager observed two Aides supervising a group of 14 Children between the ages of 3 and 5 years old on May 12.
LS observed qualified Staff supervising all groups in all classrooms on May 11. LS Manager observed 2 Aides supervising a classroom of 14 children between the ages of 3 and 5 in room 9 on May 12.
LS observed a group of 20 children between the ages of 18 months to 2 years old on the playground on May 12. LS Manager observed a group of 18 children between the ages of 18 months and 2 years old in the playroom, classroom 2 on May 12.
LS observed one Teacher alone supervising 15 children between the ages of 3 and 5 years old in room 3 on May 11.
LS Manager observed staff on the playground not stationed among the children for immediate intervention can occur.
Center self reported video footage of a Staff member yanking and pulling the arms and legs of, roughly dropping, and forcefully holding down a 3 year old child. The incident in the video occurred at 12:40pm on April 29, 2026. During LS's follow up conversation with the Center on April 30, 2026, Center reported that Parents of the child had not been notified of the incident as of April 30, 2026.
