LS did not observe a complete OCC personnel list of staff.
Program staff stated the infants have occasionally been in strollers on walks in the gym during closing. The toddler and 2's room are separated by tables in the gym. One staff stated she saw a toddler child be knocked down by a 3/4y yr old preschool child who ran through the gym while the 3/4 yr old class passed through the gym to go outside. Program staff stated the 3-5 yr old children are on the playground during closing time and use the gym at a different time in the evening than the infants and toddler room. Program staff stated each group of children are with teachers and there's been a total of 16 children at on time in the gym separated by age group. Program staff stated the gym is used occasionally during closing time. LS informed the program to properly secure all of the children if there are older ages groups passing through the gym to get to the playground and to ensure toddler, infants, and 2s are safe. Program will discuss with closing staff to use a different gym door for the 3-5yrs olds to pass through to prevent the 3-5 year old children from walking through the gym.

LSs observed 4 emergency forms that need to be updated by the parents. Program is giving every parent an emergency form to update for the start to the school year.
LS observed 2 children that need Health inventory Part I and/or II completed. Program will ensure that all medical forms are updated for the new school year.
LS observed 3 children files that need lead test completed. Program will request updated records from parents, immediately.
LSs observed out of date staffing patterns. The program will update the staffing patterns, immediately.
LSs observed 5 infants in the toddler room. Program was considering toddlers to include 12 - 24 months. Program will immediately correct the group size to meet regulation of no more than two infants in the toddler room with a max of 9. The program states that they may move the older toddler to the Two's room, and 2 infants to the infant room. The program will send corrections including staffing pattern to LS.
LS did not observe written individual activity plan for each infant and toddler. Program will correct this immediately.
