This mandatory review inspection recorded no violations or advisories.
Licensing Specialist confirmed from self-report of facility that a child left the building and walked home unaccompanied. Facility needs to submit letter of corrections stating their policies that are in place to prevent this from happening again.
Licensing Specialist confirmed a child left the facility on May 12, 2025 and staff were unaware that the child was missing until the parent notified the Director via email. Facility needs to submit letter of corrections stating what practices they are implementing to prevent this from happening again. LS recommends that all staff complete additional training in active supervision.
Licensing Specialist observed two children enrolled on February 12, 2024, who did not have the required forms, except for their Emergency Form on file at the center at the time of inspection. Facility needs to obtain Health Inventory, Lead Testing Certificate, Immunization Record, and Parent Agreement for each child, or disenroll.
Licensing Specialist observed a substitute working at the time of inspection who is not on the facilities Personnel List and no file was on site at the time of inspection. Facility to submit staff member's required paperwork if being added to the site.
