There is one child who is missing the immunization records. While on site, this was received from the parents and placed into the child's file.

The medication for the child was not present at the center. While the licensing specialist was present the parent brought the medication on site.

In room 5 the staffing pattern was incorrect with having an aide listed by themselves. This was corrected on site and a copy of the current staffing pattern given to the licensing specialist.
Along the metal fence there are large wooden boards with a metal piece and tire that must be removed or blocked off from the children to remain safe. Also, along the wooden fence between the concrete and grassy area there is a plastic tarp with chains that must be removed as well.
In room 5 there was an aide alone with the children. This was corrected on site when the teacher came back into the room while the licensing specialist was present.
