Children in care were missing the following information on the emergency cards: pediatrician information, hours and days of expected attendance and enrollment date on the emergency cards. The center does not have a current fire inspection. The fire inspection in the file is dated March 2023. Director has called the fire department who stated that they are behind on inspections.
Infant schedules were not completed for any of the infants in care. The Director was unaware of what Licensing was referring to. Licensing reviewed with the Director at this inspection activity plans for infants and children.
Activity plans were not present for the infants.
Not all of the attendance records have the children signed out at the end of the day. The attendance log was not in chronically order. One sheet had 1/30/2025 on one side and the other side had 12/18/2024, 02/04/2025 and 02/28/2025. Licensing informed the Director to make sure that the children are being signed in and out everyday.
Fire drills had not been completed in 2025. The last fire drill was December 23, 2024.
None of the contracts had the fees for care listed. The Director was able to show Licensing the form on her computer for the parents which listed the rate of care. Licensing informed the provider that it needs to be stated in the contract or attached to the contract.
One child in care was missing the immunizations. One child in care was missing the health inventory pages 1 & 2.
Emergency cards were missing the second page of the emergency card, parent's name and information, daily pick up person, complete pediatrician information and a date from when the parent signed the emergency card.
Lead screenings were not complete. Four children in care were missing the lead screening.
Staff orientations had not been completed for 3 staff.
The outside area by the back porch has a raised jagged patch of concrete that could be a trip hazard. Please have this area fixed and submit a picture to licensing.
Staff were missing the staff orientation in their file.
The infant/toddler teacher has not completed 12 hours in her anniversary year. The teacher's hire date at this center is 4/17/2023. The infant toddler teacher completed 9 hours of training in 2023, 1.5 hours of training in 2024 and has completed 6 hours in 2025. Please complete training and submit on the professional development plan along with the certificates attached.
