A 2yo child was left outside and unattended for 5 minutes.
LS viewed a video in which staff member SC forcefully grabbed a child by the arm.
Appropriate discipline was not used by staff.
Forcefully grabbing a child by the arm is not consistent with the requirements of this chapter.
SC was observed using physical discipline on video.
A 4 year old was left on the playground alone by two teachers for approximately 1-2 minutes.
A 4 year old was unsupervised on the playground because the teachers failed to complete the necessary "name to face check".
Staff allowed prescription pills to remain on the counter in the infant room. While taking the pills approximately 12 minutes later the staff dropped a pill on the floor. The pill was discovered on the floor by an 11 month old approximately 80 minutes later. The child ingested the pill.
Staff allowed the parents of 2 infants to enter the room and place their children on the floor and for at least 12 minutes the staff did not observe or interact with the infants.
Staff allowed individual prescription pill to remain on the counter in the infant room for 12 minutes.
A teacher was observed pulling a 2 year old child by the arm.
Several classrooms are in need of repair. The repairs made to vinyl baseboard in several classrooms is not acceptable. The vinyl baseboard must be replaced.
