At the time of the visit the wooden playground climbing structure table is splintering, the wooden balance beam is not sturdy, the table cloth on the children wooden picnic table needs to be replaced, and the umbrella stand needs to be thrown out.

In the {REDACTED} classroom the children's milk and food was not being stored in a manner to keep the food cold and bread was left unwrapped on top of the cubby.
At the time of the visit the following was not in good repair: The molding in {REDACTED} classroom was peeling off and the door under the sink in was not attached at the bottom, the outlet cover in {REDACTED} is broken, and the cabinet door was broken.
At the time of the visit in the {REDACTED} classroom there were some cleaning supplies under the sink and in the {REDACTED} classroom an educator's purse was accessible and within reach of the children.
At the time of the visit (1) medication consent form was expired.
