At the time of the visit, accurate attendance was not maintained in two classrooms as arrival time was not documentation for a child in a {REDACTED} classroom and two additional children in the {REDACTED} classroom.

1. Documentation of an updated developmental history form, first aid emergency medical consent and release form and transportation plan were not available for review in two children records. 2. Documentation of an updated physical examination and lead screening was not available for review in a child’s record file.
1. Documentation of an updated physical examination and an annual performance appraisal/evaluation were not available for review for two staff members. 2. Documentation of an interview was not available for review for three staff members. 3. Documentation of the EEC 12-hour Strong Start Essentials training certificate was not available for review for a staff member hired on 5/10/2024.
An updated Building Inspection was not available for review, last inspection on file recently expired on 12/31/2024.
Various classroom spaces and a restroom used by children, reflected peeling of paint on various walls. The main entrance door reflected some peeling paint.
At the time of the visit, documentation of emergency evacuation practice fire drills for the months of January 2024, September 2024, October 2024 and December 2024 were not available for review.
A child's individual health care plan for a chronic medical condition was outdated as of 8/28/2024.
Two prescription medications (Epi Pens) were expired as of 11/2024, and an additional prescription medication (inhaler)was expired as of 3/31/2023.
