Violation
At the time of the visit, it was observed that the preschool students were split into three groups with one educator responsible for each group. The group had two main attendances, with thirteen students signed into each attendance. At the time of the visit the program was not operating in two groups of thirteen but rather two groups of eight an one group of ten. The program must maintain a method of knowing exactly who is present on the premises at all times including when in small groups. During a meal time, it was observed that 26 Preschool students were grouped together. The maximum ratio within preschool is two teachers to twenty students. Within the second Toddler/Preschool classroom, it was observed that the classroom had a gate separating the room in half. An Assistant Teacher stepped over the gate, leaving a Teacher alone with nine students eating. When identified by EEC, the Assistant Teacher rejoined the main group. The program must maintain proper teacher to student ratios and ensure adequate supervision of the group at all times.

Violation
At the time of the visit, in a review of two students IHCPs the documentation was not fully complete. Both plans were missing documentation of educators trained on the health conditions. One student was prescribed two medications for their health condition, the 'side effects of the treatment' was listed for one prescribed medication, however it was unclear which medication it was referencing and which medication did not have 'side effects' documented. Another student was missing documentation of the 'description of chronic health care condition', 'symptoms' and 'potential consequences if treatment is not administered'. The licensee must maintain as part of a child's record, an individual health care plan for each child with a chronic medical condition, which has been diagnosed by a licensed health care practitioner. The plan must describe the chronic condition, its symptoms, any medical treatment that may be necessary while the child is in care, the potential side effects of that treatment, and the potential consequences to the child's health if the treatment is not administered.
