Several water bottles were not labeled with the name of the child for whom they are intended.
At the time of the visit, the staff record checklist was not current, as several staff members had no physical date or annual evaluation dates recorded. Please ensure the staff record checklist is up to date.
Two staff records contained insufficient evidence of teacher qualifications. All educators must possess the qualifications required for their positions in accordance with EEC regulations.
A review of three staff records found insufficient evidence of ongoing bi-monthly observations. Additionally, one record lacked documentation of a current physical and adequate proof of MMR immunity.

The insulation covering of a heating pole in the {REDACTED} classroom was broken, exposing the insulation material.
A review of the evacuation drill log reflected no evidence of fire drills conducted for the months of January, July, August and December of 2024.
The use zones under and around swings and the slides structures were not covered with an adequate depth of impact absorbing material.

The floor heat register in the {REDACTED} classroom was exposed in several areas presenting a health and safety concern. This item has been addressed with the program on multiple occasions.

At the time of the visit, the children's record checklist was not available for review. The EEC licensor requested to review two records; the child with a medical condition and an {REDACTED} that was present during the visit. One record did not contain evidence of current physical and {REDACTED}, while the other record did not contain evidence of periodic progress reports.
There was no evidence of current first aid certificate for four staff members.

On 3/23/22, EEC observed: "in the {REDACTED} room, the floor heat register was exposed in several areas, a health and safety concern." On 4/6/22, the program submitted a corrective action plan to EEC stating, "the heat register is no longer exposed. It has been temporarily covered. A permanent covering is on order." During today's visit, EEC observed that the floor heat register is still exposed, presenting a health and safety concern. The program did not adhere to the corrective action plan submitted on 4/6/22.
At the time of the visit, five children were in the {REDACTED} classroom; however, the daily attendance record did not indicate their arrival time.
The staff schedule was not posted nor available for review at the time of the visit.
The documentation of the monthly fire drills was inconsistent. A few fire drills were documented but did not include the year it was completed.
The individual health care plan for a childcare child was not available for review.
This investigation inspection recorded no violations or advisories.
