At the time of the visit, the health care consultant agreement was not current.
The children's record review reflected the following: 1) One record contained no evidence of current physical. 2) A second record contained no evidence of annual lead screening. 3) Two records contained no evidence of current physicals and lead screenings.
The staff record review reflected several records with no evidence of bi-monthly observations on file.
At the time of the visit, the staff record checklist was not up to date. Please ensure that all sessions are completed and are up to date.

1) Children used hand sanitizer instead of washing their hands with soap and running water before eating their lunch. 2) The tables were not disinfected before lunchtime.
