At the time of the visit, the health care consultant agreement was not current.
The children's record review reflected the following: 1) One record contained no evidence of current physical. 2) A second record contained no evidence of annual lead screening. 3) Two records contained no evidence of current physicals and lead screenings.
The staff record review reflected several records with no evidence of bi-monthly observations on file.
