Some of the enrollment records were missing some of the required signatures. 4.) The general permission of the enrollment packet is incomplete for some of the records. 5.) One record is missing the annual update.

Six of the children's records were missing some of the required information. Some of the records were missing annual updates, updated physical and lead exams, and parent signature on some of the areas of the records. The educator must confirm this information is up to date.
The CPR and FA certification is expired. The educator was informed this must be completed as soon as possible.

The educator added a fence to the outdoor space. The wood side area to the fence has broken panels. This must be replaced and in good repair. The concrete pole next to the fence and two of the concrete structures connected to the legs of the swing playscape must be protected with a barrier in order to prevent injuries. The home has a trampoline that must not be used with the day care children. The gate to the swimming pool must include a lock to prevent the children from having access to the swimming pool. Please provide pictures of corrections.
