One child's medication consent form was not complete. It was missing the parent's signature and date. Another child's medication consent form was not current (expired 4/5/2025). A third child's medication consent form had two medications listed. Each medication must have its own medication consent form with instructions, storage, dosage, ect.

The following was observed to be accessible to children: In PSA, an open box of gloves and a tube of Aquaphor which has the label "Keep out of reach of children" sat on a low shelf outside of the bathroom. The educator placed these items out of reach during the visit. The licensee must only use indoor and outdoor equipment, materials, furnishings, toys, and games that are appropriate to the ages, needs and developmental level of the children enrolled. They must be sturdy, safely constructed and installed, non-tippable, flame retardant, easily cleaned, and free from lead paint, protruding nails, rust, and other hazards that may be dangerous to children.

The following was observed to be accessible to children: In the Toddler Area, two free standing basketball hoops were not secure and tippable. The outdoor play area must be free from hazards including but not limited to: a busy street, a parking lot, poisonous plants, water hazards, debris, broken glass, chipping, peeling or flaking paint, dangerous machinery or tools, and weather related and environmental hazards or small objects that could present a choking hazard to young children. Any such hazard must be removed or fenced by a sturdy, permanently installed barrier which is at least four feet high or otherwise protected or removed, as appropriate.
