The evacuation drill log documentation for the past year was reviewed. It was noted that documentation did not include 1) the number of children evacuated; 2) the effectiveness of each drill and 3) route used.
Three children's files were reviewed. 1) One child's Individual Health Care Plan did not include the staff who have been trained in administration of the medication and who trained the staff. 2) One child's file did not include a current [REDACTED] physical exam and evidence of a [REDACTED] test.
