Safe Sleep
Educator did not have attendance available and accessible for Licensor to review. Please ensure attendance is taken daily and the sheet is always accessible; confirm this will be done moving forward.
There were no hazards observed in the licensed space. Licensor walked through new room Educator is looking to license. There were stacked bins that should be taken down to ensure items don't fall on children. There was one cabinet that needs to be anchored; please anchor the furniture and confirm once completed. The room also had excessive items that should be cleared to ensure the children can move freely through the home. Please upload pictures as confirmation.
There was an infant sleeping in an unlicensed room when Licensor arrived. Please ensure children only occupy licensed space and confirm this will be done moving forward.
There was a 9 month old infant napping at the time of this visit. The pack and play contained multiple blankets, a teddy bear and a toy hanging on the side of the pack and play. For this violation, Educator will need to sign the Safe Sleep Pledge, have parents sign the safe sleep letter, and complete the Strong Start Safe Sleep training. Once all tasks are completed, please provide documents to Licensor and complete training within 7 days of violation; confirm once completed.
Licensor reviewed children's records and some were missing up to date lead tests and permissions. Please review records and ensure the ones that they are complete; confirm once completed.
First Aid Kit
Licensor walked through indoor space and there were no hazards observed. In outdoor space, there was a small trampoline, which are forbidden. Please remove trampoline from the space and upload a picture once completed.
