Outdoor Playspace - Hazards
Licensor arrived to the home and observed a 3 year old child picking up his bicycle on the sidewalk alone; he was a considerable distance from where Educator was standing in her garage (Licensor could not see her), which could have resulted in him going into the street or gaining access to railroad tracks that are adjacent to the home. Please always utilize good judgement when supervising the children and ensure they are accessible to you to avoid injuries or incidents; confirm this is understood and will be done moving forward.

Licensor arrived to the home and saw children riding their bikes on the sidewalk and driveway. It has been discussed with Educator in the past that the children must remain in fenced in area when playing outdoors as Educator's property is adjacent to a railroad track. Please ensure children remain in fenced in play space when outside to ensure they don't have access to the railroad. Please confirm this is understood and will not occur moving forward.
During this visit, there were several instances where the Educator did not remain on the same floor level with the children. There were two instances which she went upstairs without the children and one instance where she sent an older child upstairs to use the restroom alone. Please ensure children always remain on the same floor level as Educator and confirm this will be done moving forward.
Licensor walked through indoor space and there were no hazards observed. Licensor walked through outdoor play space and noted that the fence was not stable and overall flimsy. Fencing has been cited in the past and continues to be a concern as Educator resides right next to a railroad track. Please reinforce fencing or build a sturdier fence to ensure children cannot gain access to the railroad tracks. Confirm once completed and a visit will be scheduled.

Licensor walked through indoor, licensed space and there were no hazards observed. Licensor walked through outdoor space and the following hazards were observed: The fence was unstable and also had areas which had sharp wire exposed. Furthermore, the gate to the fence was unstable, becoming detached, and had spacings of more than 4 inches, which is a head entrapment hazard. Please ensure fence is made free of hazards, is stabilized and properly attached. Also, add a mesh to the gate to avoid head entrapment; confirm once completed and a follow up visit will be scheduled.
