During the Health and Safety Inspection of the program, the following was observed: 1) An educator in the Magnolia Classroom did not wash their hands before handling food. 2) An educator in the Cherry Classroom did not wash their hands after wiping a child's nose.
During the Medication and IHCP review, the following was found: 1) One child's medication was not in the containers in which it was originally dispensed, and did not have the original prescription label.

During the Safety inspection, the following was found: 1) Cords were found hanging down from the window in the Maple Classroom. This was mentioned during the program's last monitoring visit.
At the time of the visit, the railings entering the front building were found to be rusting and chipping paint. This was mentioned during the last visit to the program.
Disinfecting wipes were found within the reach of children in the Preschool Classroom.
This investigation inspection recorded no violations or advisories.
At the time of the visit, the Cedar Classrooms' exit/evacuation route needed to be clear of obstructions. The diaper pile and chair were found blocking the classroom's emergency exit.

Five children's records were reviewed, and the following was found: 1) Two children's files did not have evidence of current updated paperwork. 2) One Child did not have evidence of a current Physical.
During the Safety inspection, the following was found: 1) Cords from the outlet were found hanging down on the changing diaper in the Cedar classroom. 2) Two rugs were found rolling up, causing a tripping hazard for the children in the Cedar classroom. 3) In the Oak classroom, the foam rugs were found to be splitting open, peeling, and exposing the foam. 4) Chipping paint was found on the blue shelf in the Oak classroom. 5) A chair in the Oak classroom was chipping paint and had exposed rust on one of the chair legs. 6) Chipping/peeling paint was found in the preschool bathrooms. 7) Cords were found hanging down from the window and the shelf by the bathroom in the Maple Classroom.
Review of two staff files found the following: 1) One staff member did not have evidence of a staff form. 2) Two staff members did not have evidence of verifications of two references. 3) One staff member did not have evidence of the date of hire. 4) One staff member did not have evidence of a current EEC PQ Registry. 5) One Staff member did not have evidence of[REDACTED]6) One staff member did not have evidence of a completed 2.0 EEC essentials certificate. 7) Two staff members did not have evidence of a completed program orientation.
During the IHCP/Medications review, the following was found: 1) One of the children's medications was found expired in the Maple classroom.
In the Maple Classroom, disinfecting wipes were found within reach of the children.

During a review of the program's indoor space, the following was learned: 1) toilet paper was not stored in a safe and sanitary matter when it was stored on top of the toilet, rather than in the dispenser 2) Although the program reported that they are waiting for the work to be complete, there were stained ceiling tiles in the children's bathroom in "cedar" and in the second toddler room, creating a potential hazard 3) the classroom mini and large refrigerators were not secured as required, creating a potential danger.

2 staff records were randomly selected for review and the following was learned: 1) Educator A, (start date 1/15/2024) does not have written evidence of supplies the program with a physical within one year as required.
4 children's records were randomly selected for review and the following was learned: 1) 2 children did not have evidence of a current face sheet, first aid and emergency consent form, transportation form, developmental history form and off-site consent form as required by regulation.
During a review of the program's three playground areas, the following was learned: 1) the mulch under the "big climber" on the preschool side was "too large" considered to be both a safety hazard and a choking hazard 2) Although the program reported that they were waiting for the work to be completed, it was observed that the basement window that is overlooking the toddler playground was cracked, accessible to children and creating a potential danger.
Six children's records were randomly selected for review, and the following was learned: 1) Two children's paperwork expired. 2) Two children did not have evidence of a Progress Report.

In the Infant Classroom Refrigerator, one child's four food containers were not labeled with the child's name.
Three staff files were randomly selected to review, and the following was found: 1) One Staff Member did not have evidence of a resume or an interview date. 2) Three Staff members did not have evidence of verification of 2 references. 3) One Staff had an expired PQ Registry. 4) One staff did not have evidence of a current Physical or[REDACTED] 5) One staff had no evidence of completing the EEC Essentials Training. 6) Three staff members did not have evidence of bi-monthly observations.

During the Evacuation Drill log review, the following was found: The program did not complete Evacuation Drills in December 2022, January 2023, and February 2023.
Multiple rest mats were worn out and ripping/tearing in the Cedar and Maple Classroom.
During the safety inspection, the following was found: 1) In the Magnolia, Cedar, and Maple Classrooms chipping and peeling paint and holes in drywall/plaster were found on multiple walls. Both create a potential hazard for the children. 2) The rug and circle mat in the Infant Classroom had multiple rips.

The magnolia classroom has insufficient materials/toys and no variety of equipment for the number of children present for daily activities.
In the oak classroom, bleach was found within reach of children in an unlocked sink cabinet.
In the Oak classroom refrigerator, two food containers were not labeled with the child's name.
The following was found during health and safety inspection: a handle to the sink in the Oak classroom was broken, rugs in the Magnolia and Cedar classrooms had multiple rips, multiple outlet covers were missing in the Cedar classroom, the dramatic play kitchen in the pine classroom was not secure, a screw was sticking out from the bathroom sink vanity on Maple classroom side, and both end metal caps to the baseboard in bathroom between Pine and Maple were removed and on the floor. .
The following was found during health and safety inspection: a bookshelf in the Oak classroom was not secured on both sides, mirror-like material is peeling off of a piece of equipment in the Oak classroom, dollhouse in Cedar classroom was not secured, a rug in the Pine classroom is a tripping hazard, a broken toilet and light was out in the Pine classroom bathroom, and the top of diaper changing table in Cherry classroom needs to be secured.

In the Cedar Classroom, diaper creams were found to be accessible to children and pose a potential hazard.
In the infant classrooms it was learned the sink is not being cleaned and disinfected in-between bottle warming/food prep and handwashing.

Arrival time was crossed out next to a child's name on the attendance roster. It could not be determined when the child arrived to the program.
Not all cups and food items were found labeled individually for the child they were intended for.
When a new educator was asked if any children had emergency medications they could not clearly explain that a child had a food intolerance with no medication.
This follow-up visit inspection recorded no violations or advisories.
During health and safety inspection the following was found: an educator left a cup with coffee on the edge of a cubbie within reach of children, multiple broken blinds found in classrooms and a diaper mat had a tear on its surface.
Licensor observed an educator spray soap & water and bleach & water on the diaper changing table without wiping the surface clean after the soap & water. Proper 2-step process to clean and disinfect was not completed.
A child's IHCP did not have clear instructions. REDACTED] was crossed out 2x on the IHCP form, but written in on the bottom. It was not clear when the "Antihistamine" was supposed to be given.
Sand on the I/T playground needs to have weeds removed and have the sand rototilled.
Licensor observed an infant in a crib with a "wubbanub" (a pacifier with an attached stuffed animal). Infant is under 12 months old. This is the program's 2nd safe sleep violation.
Educators in the preschool room were not positioned near the bathroom to adequately supervise the children in the bathroom.

This follow-up visit inspection recorded no violations or advisories.
Inspection of playground found a sensory table with wooden cover that is splintering and hazardous.
Two safe sleep violations found when Licensor entered the Infant Oak classroom. An infant was found asleep in a bouncy chair. Educator stated the infant had been asleep for a few minutes. Licensor had to instruct educators that infants must be moved to a crib immediately after falling asleep. Licensor also found the program has been using a Merlin's Magic Sleep sack on an infant and these sleep sacks are not allowed.
When educators were asked in a toddler room if there were any emergency medications they were not clear whether there were any. One educator said they were new and just getting familiar with everything.

Random review of 5 staff files found: one staff person's first aid card does not state "pediatric" on it and their doctor's note on file states "resume to prior work duties" and is not considered a physical. Administrators not documenting verbal references before hire. Administration is not conducting observations of all staff at least every other month.
Health and safety inspection of indoor space found the following: bottle of bleach solution within reach of children, metal on baseboard heater in preschool is falling off, cork wall blocking the window area in preschool is broken, broken tiles found in Cedar room and on stairs to 2nd floor, bug spray & sunscreen found within reach of children, educator's purse hanging within reach of infants, and although gate in infant room was secured to back of diaper changing table, the back surface is not strong enough to hold hooks securely.
Mulch that has been installed on the playgrounds is not appropriate and poses potential hazards. The mulch contained large splintering stick pieces and choking hazards. There was also not an adequate amount of mulch around the playground climbing structure.
Required allergy/medical conditions/emergency medication posting not posted in a toddler classroom.
When asked how they cleaned the diaper changing table after use an educator stated they used soap and water and then bleach and water "only if a poop diaper" but only bleach and water after a wet diaper.
Either no consent or an expired medication consent was found for 2 children's medications.
Not all children's water bottles and pacifiers labeled for the child they are intended for.
A child's IHCP states REDACTED] as part of their emergency plan and there is none at the program. Another child's IHCP expired 1/26/22.

While reviewing staff files it was learned the wrong BRC consent form has been used.
Review of 3 children's files found the following: 1 did not have a current [REDACTED], 1 did not have a developmental history and 1 did not have current [REDACTED] and their developmental history was not signed/dated.
Allergies, chronic medical conditions and emergency medications not posted in classrooms.
Review of 3 staff files found the following: 1 file had documentation of only 1 verification of reference, and 1 file had no evidence of Strong Start training, no doucmentation of start date and was hired without a current [REDACTED].
A teacher was observed leaving their classroom and going to another classroom to get supplies, leaving their classroom unsupervised with no other staff present.
During the visit it was learned that staff in the infant classroom warm water in the microwave, then put the heated water in the sink to warm bottles which poses a potential hazard to children.
It was learned snack provided consists of either a fruit or a vegetable. This does not meet USDA guidelines.
Toothbrushes were not found stored in a safe and sanitary manner. Wooden holders had toothpaste residue on them, some toothbrushes were found touching and some toothbrushes found with dried toothpaste in the bristles.
A cracked mirror was found in PreK room which poses a potential hazard.
It could not be determined if evacuation drills are completed monthly with all groups of children and educators.

It was learned staff are not cleaning and disinfecting with a two step process. One teacher stated they use soap and water on tables prior to children eating and bleach and water on the diaper changing surface. Another teacher stated they used Simple Green and when the bottle was reviewed it stated it was only a cleaner. A basket of dirty tissues was found in a wicker basket on a shelf in the infant classroom. The basket cannot be properly cleaned and disinfected.
