Licensor randomly reviewed 4 of the children's records. Of these, 1 was complete and up to date, but the other 3 records all had out of date lead testing.
There is a child in the program who is prescribed an epi-pen. The epi-pen that is stored in the program expired in 11/2024. Tested the smoke detector and the carbon monoxide detector in the kitchen and both worked properly. All of the required items were observed in the 1st aid kit.

Licensor inspected all of the indoor, licensed space, as well as the outdoor playspace (fenced in back yard). The only hazard that was observed was in the kitchen. The top drawer on the far right of the counters has screws and other small chokable items inside it.
Indoor Hazards
Licensor randomly reviewed 5 of the children's records. Of these, 2 were complete and up to date, 1 had an out of date [REDACTED], 1 had no signature on Pg.8 of the enrollment packet under General Permission - Basic Transport, and the last record had out of date signatures in the enrollment packet and an out of date [REDACTED]. Please review all of the children's records and ensure they are all complete and up to date.
Licensor inspected all of the indoor, licensed space during the renewal visit. The only hazard that was observed was in the kitchen. Educator has a child proof lock for the cabinets under the sink. However, this lock was not engaged when Licensor inspected the kitchen, and there are cleaners/chemical in those cabinets.
Licensor randomly reviewed 5 of the children’s records. Of these, 3 had complete and up to date progress reports, but the other 2 progress reports were out of date.
The temperature in the refrigerator was 42F degrees. Regulations state the temperature must be between 32F-40F degrees.
A fire drill has been conducted every month so far this year, most recently on 3/4/25. However, the same exit has been used every month. Discussed that the exit used during the drills needs to be alternated.

Licensor inspected all of the licensed, indoor space during the monitoring visit, as well as the outdoor playspace. The following hazards were observed: **PLAYROOM** - no hazards **KITCHEN** - there were sharp knives on the counter **CLASSROOM** - no hazards **BATHROOM** - no hazards **BASEMENT: PLAYROOM 1** - power strip with exposed outlets **PLAYROOM 2** - no hazards **OUTSIDE** - no hazards.

Licensor reviewed 5 of the children’s progress reports and they were all out of date.
Licensor inspected all of the licensed, indoor space during the monitoring visit. The following hazards were observed: ** PLAYROOM - tall shelf with toys on it needs to be anchored to the wall as it is a tipping hazard ** KITCHEN - drawer to the right of the stove has a pizza cutter and a lighter in it, top drawer on the other end (next to the table) has a Swiss Army knife, screws, and other small chokable items ** CLASSROOM - no hazards ** BATHROOM - no hazards ** BASEMENT: PLAYROOM 1 - outlet not covered ** PLAYROOM 2 - no hazards ** OUTSIDE - no hazards.
Children’s coats and bags are hung on hooks by the back door. Their belongings are not far enough apart and touch each other, which is a cross contamination concern. These items need to be stored so that they are not touching.
Licensor reviewed 5 of the children's records. Of these, 4 were complete and up to date. The 5th record had out of date signatures in the enrollment packet.
Licensor discussed with Educator the need for a list to be posted of all emergency or lifesaving medications, including but not limited to epinephrine auto-injectors, inhalers, and anti-seizure medications that specifies to which child they belong. Licensor also discussed that this information needs to have a cover sheet in order to protect the child’s confidentiality. There is one child in the program who is prescribed an epi-pen and this information needs to be posted.

Extra clothing & assuring children are dressed appropriately for the weather
Educator has daily attendance forms and the parents are responsible for signing their child in and out and writing the time of arrival and departure each day. There were several days in which the parents did not sign out their day at the end of the day, so there was no departure time.
Licensor inspected all of the indoor, licensed space during the renewal visit, as well as the outdoor playspace. Inside, the only room that had hazards was the kitchen. The cabinet under the sink has a lock but it was not engaged during the visit and there are cleaners inside, the top drawer on the right side of the counter has small, chokable items inside, and there was a large butcher knife in a utensil canister on the counter.

Educator has a completely fenced in back yard, which she utilizes as the outdoor playspace. There was a climbing structure that appeared to be approximately 4 ft high. This had no fallzone underneath it, other than the artificial turf that is reportedly 4 inches deep.

Attendance Logs
Licensor reviewed 2 of the children's records during the virtual visit. One record was complete and up to date. However, one of the parent signatures was in the wrong place and Educator will need to ensure this is fixed. The other record had no signature for receipt of the parent handbook.
Educator uses the attestation forms as the attendance. The parents sign the child in at arrival and sign them out at departure. These times are documents on the attendance. However, the parents do not always write in these times, so it is not consistent. Discussed that Educator needs to ensure these times are documented every day for each child.
Educator has a detailed emergency preparedness plan that is posted in the program. However, it is missing Educator's plan for what to do if a child goes missing from the program.
This monitoring visit inspection recorded no violations or advisories.
