During the visit, one grouping did not maintain an accurate staff-to-child ratio. One educator was found with fifteen children.
During the Medication review, the following was found: 1) One Children were missing required Medication.

An [REDACTED] was found expired as of 4/2024.
Adequate supervision is not provided to children walking out of the cafeteria to go put things in their backpacks. Multiple children were observed walking out of the cafeteria unsupervised and educators were unaware.

At the time of the visit, the program did not have evidence of two children's IHCP.
