During the visit, the following was observed: In multiple classrooms, trash containers were found to be uncovered.
Indoor Space
During the safety inspection of the program, the following was learned: 1) Chipping and peeling paint was found in the PreK-A classroom. 2) In the PreK-A classroom, educators' bags were found within the children's reach.
At the time of the visit, the following was observed: 1) One Educator in the PreK-A walked back into the classroom leave six children in the hallway and in the bathroom unattended. 2) One educator left the PreK-B classroom to bring one children to the bathroom down the hall leaving the other educator in the classroom with eleven children. 3) One educator left the Preschool classroom leaving the other educator with fourteen children during stem.

During the Medication/ IHCP review, one child did not have the required Medication listed on their IHCP in the Pre-KA Classroom.
Two Staff Records were randomly selected for review, and the following was learned: 1) One Staff member did not have evidence of a current physical. 2) One Staff needed evidence of [REDACTED] One Staff Member did not have a completed EEC Essentials Certificate.

Three staff records were randomly selected for review, and the following was learned: 1) One staff did not have evidence of a current Physical as required by regulation. 2) One Staff did not have evidence of the EEC Essentials Certificate.
Outdoor Space - R Apr 2025
During the safety inspection of the playground, the following was found: 1) The rug under the shade structure was rolling up, creating a tripping hazard. 2) The wood on one of the red garden beds was broken and falling off.
During the safety inspection, the following was found: 1) Multiple shelves and dramatic play equipment were not secured throughout the program.
Toxic Substances
At the time of the visit, one child's prescription medication in the Pre-K B Classroom was not in the container in which it was originally dispensed and didn't have the original label.

At the time of the visit, the following was found: 1) Hand sanitizer was within reach of children in the Pre-K A and B Classrooms. 2) Disinfected Spray bottle was found on the red shelf in children's reach in the Preschool Classroom.

One preschool classroom had not taken attendance for the day and the toddler class had 4 children and 3 children were not signed in on the attendance roster.
