The Bunnies Infant/Toddler did not have an art or sensory areas. The Piglets Toddler classroom did not have art supplies available to the children.
Through a review of the staff record's checklist, it was noted that one staff member did not have record of MMR vaccinations, and 2 staff members had expired physicals.
The Bunnies' toddler classroom did not have adequate lighting during rest time to ensure appropriate supervision.

Through a review of the children's record checklist it was noted that two recently enrolled students did not have the required immunizations records, and one student's medication consent was expired.
One students IHCP was expired.
Through a review of emergency medication consents showed one was expired.
A review of documentation of staff observations showed some staff were not being observed on a bi-monthly basis and lacked evidence of observations being shared and discussed with staff.
A review of medications showed one student did not have a medication consent for Benadryl and one student's medication consent for an [REDACTED]l were expired.

Of the six children's files pulled, the following was found: 1. One student's face sheet, transportation plan, and emergency release were expired, and the child did not have the required proof of lead test.

A review of four staff files showed the following: 1. One staff files was missing evidence of physical and [REDACTED] vaccinations 2. One staff member did not have First Aid certifications 3. One staff file was missing first aid certification and a staff form.
A shelf in the dramatic play area of the Bunnies classroom was unsecured, causing a tipping hazard.
The following was observed: 1. the child safety lock on a cabinet in the infant room was broken 2. The round rug in the Piglet room was very dirty 3. The chairs in the piglet room were dirty and had chipped and peeling paint. 4. The corner of a rug in the Bunnies room was rolled up, causing a tripping hazard. 5. A rug in the Pre-K room was lifted on the edges causing a tripping hazard.
A review of IHCPs showed that 2 were expired.
One student's rescue seizure medication was stored in a locked cabinet in the director's area therefore it was not readily available in the event of an emergency.

Some of the floor tiles in the pre-Kindergarten classroom were cracked, lifted or dented causing a tripping hazard.

* The lattice fencing on the preschool playground is broken causing a hazardous situation in which a child's hand could get caught and cut. * The preschool playground climbing structure does not have the adequate depth of mulch for the fall zone.
A review of two staff files showed the following: * Both staff files were missing the staff form, record of interview, record of reference checks, and proof of orientations. * One staff file was missing the PQ registration certificate.
A toilet plunger was observed stored in the Prekindergarten classroom within reach of children.
A review of emergency medications showed an [REDACTED] had expired in February of 2023.
A review of four IHCP's showed the following: * The required training information was missing from three of the IHCPs * One IHCP expired in September 2021.

During an inspection of the toddler classroom, the following was noted: * The large square mats in the toddler classroom were torn and frayed around the edges and corners. *The child safety lock on one of the cabinets was broken, allowing a paper cutter to be accessible to children. In the Pre-K room the cubbies and the radio shelf were not secured, causing a tipping hazard
The fall zones on the preschool and toddler playground do not have the required 9 inches of wood mulch.
During a review of the IHCPs, on was shown to be recently expired.
Upon review of a [REDACTED] IHCP, [REDACTED] was indicated as part of the allergy plan. Program did not have [REDACTED] on site.
