At the time of the visit, the Toddler Classroom did not maintain accurate attendance. Four children were present; only three were signed in.
During the safety inspection of the playgrounds, the following was found: 1. The impact-absorbing material (mulch) did not have an adequate depth on both playgrounds. 2. All weeds must be removed from the playground and the area along the fence.
During the visit, the written emergency and evacuation procedures, along with the diagram, were not displayed next to each exit in the Toddler Classroom.

One child's milk bottles were not labeled in the refrigerator of the infant classroom.
During the health and safety review, the following findings were noted: 1) Chipped and peeling paint was observed on the window sill and door frame in the toddler classroom. 2) The first licensed preschool classroom was cluttered and presented multiple hazards.
This follow-up visit inspection recorded no violations or advisories.
At the time of the visit, the program was providing care in a space that the Department had not approved. The program was found outside on the side of the building, using unlicensed space, playing with water.
At the time of the visit, Educators in the Toddler classroom did not engage in positive interactions and child guidance practices when they were over heard telling one child multiple times that their going home, that they were going to call their parents, their parents are coming and they are not going to have any fun later.

Six staff records were reviewed, and the following was found: 1) Three staff members did not have evidence of a current physical. 2) Two staff members did not have evidence of an [REDACTED] 3) Three staff members did not have evidence of a completed EEC Essentials certificate. 4) One Staff member did not have evidence of a completed program orientation. 5) Three staff members did not have evidence of bi-monthly observations.
At the time of the visit, the program was unable to ensure the safety and well-being of the children, as multiple nursing home employees without a complete EEC BRC were observed to have access to the children and the childcare space. The program is not following the plan they submitted on March 14, 2025. A more detailed plan must be submitted.
Three educators did not exercise appropriate supervision of the children in their care. Two children were observed running down the hall and into another classroom.
During the Toddler/Preschool classroom transitions from outside water play to bathroom/changing time, the Educators did not complete the transition in a safe, timely, predictable, and unhurried manner. The children were heard crying and not listening to the educators in the hallway. Educators were heard raising their voices to the children, and two of them ran away from the educators.
At the time of the visit, the program did not maintain a ratio of at least one toilet and sink in one or more well-ventilated bathrooms for every 20 children. During the visit, it was observed that the second bathroom is being used as an adult bathroom for the nursing home employee.
