A child's individual health care plan was not updated, it expired 3/11/23. Please ensure individual health care plans are updated annually.
The children's record review reflected the following: 1) a record containing no evidence of annual updates, current physical and a progress report for the month of March 2023. 2) three records containing no evidence of periodic progress reports. 3) a record containing no evidence of lead screening.
During an investigation visit conducted on 10/11/22, the program was cited for exceeding the maximum number of children allowed in the {REDACTED} playground on 9/30/22, as there were 17 children present in the toddler playground. On 12/5/22, the program submitted a corrective action plan to EEC stating, "All staff have been informed that the {REDACTED} playground can only accommodate 15 students. Staff was informed at the staff meeting and at our November PD Day,". During today's renewal visit, EEC observed 17 children in the {REDACTED} playground area, exceeding the maximum number of children allowed in the toddler playground. The program did not adhere to the corrective action plan submitted to EEC on 12/5/22.
The program failed to notify EEC of a change in the program space. The program converted two {REDACTED} classrooms ({REDACTED} #1 and {REDACTED} #2) into one {REDACTED} classroom of 20 children without any written notification to EEC. Please ensure to notify EEC of any change in program space at least 30 days prior to any change in the space used by the program.

The staff record review reflected the following: 1) A staff's orientation form did not include the staff signature. The last bi-monthly observation on file is dated 2/22/2022. 2) A staff's record contained no evidence of bi-monthly observations 3) A staff's record contained no bi-monthly observed for May 2022 and July 2022 (The last bi-monthly observation on file is dated 3/4/2022).
The children's attendance was not accurately documented: 1) The {REDACTED} Class: The attendance reflected 5 children present, however there were 6 children present in the classroom. 2) The {REDACTED} Preschool room: The attendance reflected 13 children present, however there were 15 present in the classroom. 3) {REDACTED}: The attendance reflected 11 children present, however there were 9 children present.
EEC observed a diaper cream accessible to children in a {REDACTED} classroom.
During a health and safety walk-through of the program, EEC found staff members not following the program's diapering and toileting procedures. 1) EEC observed two educators, each changing a child's diaper. The educators did not wash their hands and the children's hands after the completion of the diaper change.
