The First Aid Kit did not contain gauze roller bandage. Please ensure to maintain adequate first aid supplies including, but not limited to: adhesive tape, band aids, gauze pads, gauze roller bandage, disposable non-latex gloves, instant cold pack, scissors, tweezers, thermometer, and CPR mouth guard.
A child who has been diagnosed with a chronic medical condition by their health care practitioner did not have an Individual Health Care Plan on file. Please ensure to have an Individual Health Care Plan for children with a chronic medical condition, which has been diagnosed by a licensed health care practitioner.
An emergency medication was not immediately available. The medication was stored in another classroom far away from where the child was. Please ensure emergency medications are immediately available for use and accompany the children anytime they leave the facility in the care of staff.

At the time of the visit, EEC observed several classrooms in disrepair with chipping, flaking, or peeling paint; in several classrooms and hallways, ceiling tiles and vents were not clean or in good repair; several bathrooms were cluttered, not clean or safely maintained.
During the visit, EEC observed a fence/gate between the outdoor playground spaces which had sharp pieces of fencing at the bottom, presenting a hazard to children.
At the time of the visit, 2 refrigerators used for food storage were more than 50 degrees (Fahrenheit) in temperature. Refrigeration must be not less than 32 degrees and not more than 40 degrees.
At the time of the visit, one staff member's purse was on a counter accessible to children; one classroom had an unsecured cord/wire longer than 6 inches and accessible to children.
