Infection Control/Hand Washing
The licensor observed that during food service, a staff member answered the phone and did not change gloves before resuming food handling, which does not meet hygiene requirements.
An educator did not follow proper handwashing procedures after diapering, as the child’s hands were not washed.
A ceiling leak was observed during the inspection. Several ceiling tiles throughout the facility, including those in the preschool and toddler bathrooms, show visible signs of water staining. In the preschool bathroom, the walls also exhibit leak stains, and multiple ceiling tiles display evidence of water damage. These conditions suggest ongoing or unresolved maintenance issues and require prompt corrective action to ensure a safe and sanitary environment for children.

A teacher did not wash a child's hands before lunch. Teachers must wash hands or wear gloves before handling/serving foods. Teachers must wash hands or wear gloves after cleaning tasks and handling trash. All eating surfaces must be washed and disinfected before and after each use. Toys mouthed by children must be set aside and stored after each use and may not be used by another child until they are washed and disinfected.

The licensee must maintain adequate first aid supplies, including, but not limited to: adhesive tape, band aids, gauze pads, gauze roller bandage, disposable non-latex gloves, instant cold pack, scissors, tweezers, thermometer, and CPR mouth guard.
Toys mouthed by children must be set aside and stored after each use and may not be used by another child until they are washed and disinfected.

The inspection of the first aid kit revealed that the kit did not include tweezers on the day of the visit.

Not all enrolled children had documentation of the required yearly updates on file. Several child records were missing current physical examinations, immunization records, and lead test results. The Licensee reported that they are currently working on updating the children's files.

The allergist and medication list did not include the current children with allergies and medications. The Licensee updated the list and posted it at the time of the visit.
The staff schedule posted did not accurately reflect the staff currently working in the classrooms at the time of the visit.
The transportation attendance record was missing the signature of the secondary reviewer responsible for checking the vehicle after each trip to ensure that no children are left alone.
A second staff must conduct a complete vehicle inspection after every trip to ensure that children are not left alone in a vehicle at any time.
