Educator emailed a copy of her Parent Handbook to Licensor This handbook is very abbreviated and appears to be missing several required sections.
Licensor did not see an emergency preparedness plan posted in the program, and there is no emergency plan in the Parent Handbook that Educator emailed to Licensor.
All of the required items were observed in the 1st aid kit. However, the children’s emergency contact information was out of date. Has an [REDACTED] in the kitchen that, but no one is prescribed it

According the evacuation log, a fire drill was conducted in January and February. However, the date that the drills took place was not documented in the log.
Annual Registration in Professional Qualifications Registry (PQR)
Licensor inspected all of the licensed, indoor spaceduring the visit, as well as the outdoor playspace (fenced in side yard). No hazards were observed. However, Educator wants to add a playroom on the second floor to the licensed space. Licensor inspected that room and there was a floor lamp that was a tipping hazard.

CPR/1st Aid is up to date as it was issued on 9/17/25. However, the certificate does not state that it includes infant or pediatric CPR.

Educator has daily attendance sheets. However, none of the attendance papers the date on them.
Including the EEC Essentials 2.0 (12 hours), Educator had a total of 14 hours of trainings that were completed over the last 3 years.
Educator has a household member who she states is no longer living in the home, but is still regularly on the premises and her BRC expired in May of 2024.
None of the children’s progress reports have dates on them to indicate when they were completed, so there is no way for Licensor to know if they are up to date.
Licensor inspected 4 of the children's records. Of these, 2 were complete and up to date, 1 had out of date signatures in the enrollment packet, and the 4th record had an out of date physical and also out of date signatures in the enrollment packet. Please review all of the children's records and ensure they are all complete and up to date.
All of the required emergency numbers/information are kept on a clipboard in a bucket in the kitchen. Discussed that this info needs to be posted somewhere visible in the program.
Educator has an old epi-pen in the 1st aid kit, which does not have a label or expiration date on it. Educator stated it belonged to a child who is no longer enrolled in the program. Tested the smoke and the carbon monoxide detectors and both worked properly.
All of the required items were observed in the first aid kit, except for the children’s emergency contact forms. Smoke detector in the kitchen was tested and worked properly. Educator did not have a carbon monoxide detector on the first floor. Regulations state there must be a carbon monoxide detector on every level of the home.
Educator did not inform EEC of a change in household when her family member moved out of the home. This is a requirement, according to regulations.
Nearly a year ago, in April of 2024, Educator submitted a BRC Renewal transaction for her household member. However, this household member never completed the BRC process and was never approved. Educator reported that this family member is no longer living in the home. This information was never communicated to EEC. Licensor will be sending Educator an affidavit to attest to this so that the BRC renewal transaction can be cancelled.

There were progress reports in the children's records. However, none of the reports were dated, so Licensor had no way to know when they were completed. Discussed that these reports also have to be provided to the parents as well, which has not been done.
Licensor inspected all of the indoor, licensed space during the monitoring visit, as well as the outdoor playspace. The following hazards were observed: **KITCHEN** - trash did not have a cover **LEARNING ROOM** - no hazards **PLAYROOM** - there was an electric heater being used. Discussed that these are not allowed to be used during childcare hours. **BATHROOM** - no hazards **OUTSIDE** - 2 sections of fencing were broken, exposing the neighbors fire pit, which was not covered.
Licensor randomly reviewed 3 of the children’s records. All of them appeared to have an up to date progress report. However, Licensor could not verify this as Educator does not write the date of the report on any of them.

No immediate health risks were observed during the monitoring visit. The first aid kit was missing a roll of gauze. Tested the combination smoke and carbon monoxide detector in the kitchen and it worked properly.
Licensor inspected all of the indoor, licensed space during the visit, as well as the outdoor playspace. The following hazards were observed: **KITCHEN** - cabinet under the sink has a child safety lock, but it was not secured during the visit and there are multiple bottles of cleaners inside, a drawer in the island had a child safety latch that was not working and there were screws, nails, small tools, and multiple other hazards inside **PLAYROOM** - no hazards **LEARNING ROOM** - no hazards **BATHROOM** - no hazards **OUTSIDE** - Educator utilizes the fenced in side yard; no hazards.

Licensor reviewed 4 of the children's records. Of these, 3 were complete and up to date. However, the 4th record had out of date signatures in the enrollment packet and out of date medical information.
Educator did not have any consistent attendance. Daily attendance is required, and the time of each child's arrival and departure must be noted.
Educator has an evacuation log and has conducted a fire drill every month. However, the log that Educator uses does not track all of the required information, such as what exit was used for the fire drill. Educator also has not marked the date that the drill took place.
Educator has a certificate for CPR/1st Aid. However, it is for an online course. Discussed that an in person course is required and that this needs to be taken as soon as possible.
Educator could not locate her Parent Handbook during the renewal visit. Educator then emailed Licensor a copy of a handbook. However, this handbook was incomplete. It only contained a couple of pages multiple times, and did not include multiple required sections.
Educator could not locate the emergency preparedness section in her parent handbook.
All of the required items were observed in the first aid kit, except gauze pads.

Educator did not have copies of any of the professional development that she has completed over the last 3 years. Discussed that the requirement is for Educator to complete 10 hours of trainings each year.
Licensor inspected all of the licensed space in the home, except for the second floor bedroom. This room is no longer being used and Educator has transitioned it back to her own bedroom. The only hazard that was observed was the trash in the bathroom, as it did not have a cover.

Licensor reviewed one of the children's records during the virtual visit. This record had all of the required medical information and an up to date progress report. However, it had out of date signatures in the enrollment packet.
Licensor was able to inspect all of the licensed space during the virtual visit. The only room that had any hazards was the kitchen. There was one drawer with a child proof latch that is not catching anymore, and there were disinfecting wipes and Lysol on the countertop. Educator removed the cleaners during the visit, so that they were in the locked cabinet. None of the other rooms had any hazards. Smoke Detectors
Educator uses the bottom half of the evacuation log to track the fire drills. This section does not contain all of the required information for the fire drills.
Educator tracks attendance on a calendar. The time of arrival and departure are not noted on the calendar. This information needs to be documents daily.
Educator tested the smoke detector on the second floor during the virtual visit and it did not work. Educator then tested the smoke detector and the carbon monoxide detector in the kitchen and they both worked.

Licensor was able to inspect all of the licensed space during the virtual visit. The only hazard was in the kitchen. There is a broken child safety tab on a junk drawer, which needs to be fixed.
