Health and safety inspection found a rusty sink in a preschool classroom, faucet in infant classroom is not able to be cleaned properly, dishwasher in preschool room is not secured, and sticky paper covering bathroom door in a toddler room is peeling off.

Random review of 8 children's files found 1 without evidence of a current physical, IZs and lead test. There was no consent on file for 3 children receiving services from either EI or an ABA.
Allergy posting in classroom only stated allergies and food preferences. A child with [REDACTED] was not noted on this list.
Licensor had to remind an infant educator to wash a child's hands after their diaper was changed.
A toddler classroom had 10 children in attendance. Educator stated one child was having a difficult time in their classroom and was moved to the classroom because they are comfortable with the educators.
Random review of 10 children's files found the following: 3 files did not have parent signature/date, 1 had a signature but no date on the first aid/cpr consent, and 3 did not have a current physical.

Random review of 8 staff files found the following: 3 did not have a current physical, 2 have no evidence of [REDACTED]one educator completed the Cares Course in 2021 and has not completed the minimum hiring requirements and is being used as teacher qualified in a classroom. The work experience form on file for the same educator was not signed/dated by a qualified administrator.
Not all water bottles were labeled for the child they are intended.
Health and safety inspection found the following: white board in classroom either needs to be cleaned if possible or replaced and ensure area rugs are flat and not bunched up.
Areas of the building children have access to on the playground are peeling and broken and in need of repair.
One child's IHCP stated [REDACTED] as part of their emergency plan and there is no [REDACTED]on site for this child.

Not all bottles were individually labeled for the child they were intended for.
When Licensor entered a preschool room the children were lining up to go outside. One child was hitting another child in the head. Licensor immediately addressed the child hitting because they were directly in front of the door. One educator quickly told the child hitting to stop but no had a conversation with the two children or checked to make sure the child who was hit was ok.
No IHCP on file for a child with a chronic medical condition.
No medication consent forms on file for two children who have emergency medications at the program.
A toddler educator was observed changing a diaper and then sending the child off to play without washing their hands. An infant educator was unaware that infant's hands had to be washed after diapering.

Review of 6 children's files found 2 files with outdated enrollment paperwork.
An infant was observed laying on a boppy drinking their bottle.
Attendance record for the day of the visit reflected the infant A opening classroom was out of ratio with 8 infants and an Infant B educator was not present to immediately open the second classroom.
Review of 4 staff files found 1 file with no evidence of current first aid and 1 file with no current physical, no evidence of [REDACTED] and no completion of the infant safe sleep trainings from Strong Start Essentials training. Although this staff person still has time to complete their overall Strong Start trainings they are assigned to work in an infant classroom.
A toddler educator was frustrated with a child's behavior and was repeatedly heard saying, "I'm not dealing with this today". The educator also told the assistant that was standing near the child who was climbing on a shelf to not worry about the child because the Director said to ignore the behaviors.
During health and safety inspection the following was found: staff purses and drinks on counters accessible to children, dirty toilets and floors, stained ceiling tiles, water damage left carpet wet and damaged a shelving unit and child safety lock on a preschool room door does not work properly and needs to be readjusted or removed.
Peeling paint was found in classrooms.
When an educator was asked their cleaning process, they stated they used the disinfectant first and then used the cleaner. This is not the appropriate 2 step process.

During health and safety inspection the following was found: diaper changing mat with cracks/holes in it, holes in a classroom wall, wood tops to half walls in classroom bathrooms starting to lift, and some of the toilet and sink areas not clean.
