Equipment and Materials
Jungle Jumparoo located in the daycare room has foam matting underneath and mats covering the walls on two sides. According to the manufacturer's description, this equipment is only to be used for children 2 years old or older. The Jumaproo was observed to be used by children under the age of 2 during the visit. After this was discussed with the educator, she placed a play yard around the structure, so the equipment is inaccessible to children under 2. After reviewing photos of the Jumparoo and in discussion with the EEC Regional Director, it was decided that only two children will be approved to use the equipment at one time. Per the EEC Inner Tube policy, the educator must submit a written safety plan to the licensor. The inner tube policy was emailed to the educator with details on what information must be included in the safety plan.

Food program calendar is used to record written attendance. Observed that the attendance was not completed for October and the day of the visit. Attendance must be accurate at any given point in the day with the daily arrival and departure times of the children recorded as they occur.
Educator did not have any professional development training certificates for 2024.
Random review of five children's files. Some packets were recently filled out, but others require a signed Annual Update form. Discussed the educator going through all her files to see which child needs a form signed.

Last evacuation drill logged was in August 2023. Drills must be conducted with the children and logged each month.
CDC developmental checklists reviewed were last completed in February 2023. Other children do not have any progress reports in their files. Written progress reports must be completed every three months for infants and children with special needs, every six months for toddlers and preschoolers, and once a year for school aged children (5 years or older).
Regular Assistant [REDACTED]was working with the educator. However, when the licensor arrived, [REDACTED] was outside on a tractor. Discussed Jeffrey needing to stay indoors with the educator at all times when needed in the ratios. [REDACTED]did come in a few minutes after the licensor arrived and was in the program during the visit. Due to the fact that there were three children under 2 years present and no school age children, [REDACTED]needed to be in the home and with the educator and the children and should not have been outside.
A large standing mirror that was not secured to the bathroom counter. Discussed moving the mirror and hair ties on the counter out of reach of the children. Also discussed clutter on the kitchen island. The educator has a gate to prevent children going into the kitchen/living room area without her, but if the children are in the kitchen area the hazards on the island will be accessible to them.
Allergy list with the location of a child's [REDACTED] is not posted as required. Discussed the individual health care plan in the child's file.

Food program calendar is used to track attendance. Observed that the attendance was not completed for the day before or the day of the visit. Attendance must be accurate at any given point in the day with the daily arrival and departure times of the children recorded.
The educator had a volunteer in the program that has not undergone the EEC background record check process. The volunteer left the program during the visit.
Regular Assistant Doris Warila is no longer working with the educator. Regular Assistant [REDACTED](expires 7/12/25). Licensor reviewed a certificate for [REDACTED] during the visit but cannot find a current license for [REDACTED]in the LEAD system. Educator must email the licensor a copy of [REDACTED] certificate-letter of verification. Another caregiver,[REDACTED], was present at the time of the visit. [REDACTED] is not a licensed assistant. The educator stated [REDACTED]is a volunteer two days a week in the program, however, [REDACTED] has not been through the EEC background check process and cannot be in the program without BRC approvals, including fingerprinting. Discussed A[REDACTED]applying to become a regular assistant.
Bathroom cabinets are not locked and contain hazards.

Educator uses her own parent handbook and parents also receive the Guild of St Agnes handbook. Handbook is missing required EEC information including Safe Sleep. Educator must compare her handbooks to the EEC parent handbook template and add in any missing information.
Exit sign is missing over the front door. Written evacuation plan is posted.
Liquid soap is available on the counter by the bathroom sink. One common towel was observed with no disposable paper towels accessible. Disposable paper towels must be readily available to the children for handwashing. Discussed tearing off small sheets and placing in a basket next to the sink.
Treadmill in the playroom is not up and in a locked position. Treadmill must be unplugged at all times during daycare hours.
One child with a [REDACTED] Allergy list with the location of the [REDACTED]is not posted as required. The child's pediatrician has not filled in the Individual Health Care Plan form and the parents have not signed the EEC Medication Consent Form. The allergy list must be posted and the other forms added to the child's file.

Educator does not practice toothbrushing with the children. Parents have not signed waiver forms. These forms must be signed and placed in children's files in toothbrushing is not occuring at the program. Discussed tooth brushing without toothpaste.
Fire drill log is complete up to August 2019. Evacuations must be conducted and recorded every month.
