Radom review of 4 children’s records. Licensor observed the following: CHILD 1 was missing specific places on permission form and date of parent signature. Permission for medical treatment was missing educator's name and date of signature. Topical med was not listed. - CHILD2: Permission for topical medication was not filled out. - Physical description of the children was not filled out. - Five (5 ) emergency cards were expired.
