The licensor observed a brown shelf in the playroom that was not secured to the wall, creating a tipping hazard.

Emergency Preparedness
During the visit, 3 children records were reviewed. The following was observed: 1) Children #1, #2, and #3 did not have developmental history information in the enrollment packet. 2) Child #1: educator did not have written consent in the emergency card, general permission, and CPR/FA/medical consent in the enrollment packet. 3) Child #2: lead screening results were not up to date in the file. 4) Child #3: did not have transportation plan in the enrollment packet.
During the visit, Licensor observed pile items (boxes and totes bags that were filled with clothes and other materials) above the toilet area presenting a tipping hazard for children.
During the visit, educator did not have a parent handbook available.

During the visit, Licensor observed the emergency plan was incomplete. The plan did not included written procedures for shelter in place, lockdown, and missing children as required.
During the visit, Licensor observed various hair products near the sink in the the bathroom accessible to children.
This monitoring visit inspection recorded no violations or advisories.

There were missing evacuation logs document of the date, time, exit route used, and number children from January to May 2022.
Licensor observed next to the back door exit goes to the backyard play area have 2 big holes from the construction.
There was an empty water table in front of the back door exit.
