One child's file did not have evidence of a signed IHCP.
The paper and online attendance did not match in two rooms. When asked which would be used during an emergency evacuation, staff stated the paper attendance. This was not up to date with students who were in attendance in each room.
The large window in the front PS room (2nd building) was cracked. Although it did have tape over it, the pane could be significantly compromised. -There was chipped paint. -There were objects (vacuum, stroller, etc) lining the hallway that is an emergency exit / exit to the playground. -There was Lysol, Dawn, Resolve cleaner accessible to children in the kitchenette area on the kitchen.

In the large Preschool room, one staff stated that they had 19 children. The attendance indicated the room had 21 children and upon counting, the room actually had 20 children.
In the kitchenette area, chemicals were accessible in a cabinet under the sink. This area is accessible to children, as the bathrooms border this hallway area. -The screens for two windows in the preschool rooms were ripped.

The following were observed: -A beaded blind cord over six inches was accessible to children -A rug was curling, creating a possible trip hazard. -One shelf was tippable. -A container of ice melt, marked keep out of reach of children, was open and accessible in the hallway near the back door. -The kitchenette area had Resolve and other cleaners accessible to children. This area is directly in the walkway to the bathrooms.
In the preschool room, the posted medical information list did not include what medical condition or medication used treat the condition was needed. There was no posted list in this room that included this information.
One child was prescribed(REDACTED). The child's medication was not labeled to indicate who the medication belonged to. In addition, the child's medication consent, for this over the counter medication, was not signed by the doctor.
Outdoor hazard
The program's new playground had bolts protruding into the playground.

In the hallway to the back door, a large box, art drying rack and vaccuum were protruding into the hallway. This hallway is an emergency exit and the hallway regularly used to access the outdoor playground.
