The following was observed during the visit: two children were missing their required second (REDACTED) and another child was missing their required (REDACTED).
The following was observed during the visit, in TODDLER 3, three children's water bottles were not labeled with their names. Personal items intended for individual use by children, including but not limited to bottles, pacifiers, toothbrushes and sleeping materials, must be labeled with the name of the child for whom they are intended.

Staff names on the current Staff Records Checklist were cross referenced with names in BRC Navigator and found two current staff members do not have current BRCs.

The following was observed during the visit, a child in PreK 1 was out of the classroom for rest time and was still signed into the attendance.

Infection Control/Hand Washing
The following was observed during the visit to be accessible to children: In PRESCHOOL 1, one of the AC cords was long and not secure; in PRESCHOOL 2, one ceiling tile above dramatic play is stained, in TODDLER 3, the microwave is sitting on top of a refrigerator and is not secured (could slide off); in TODDLER 2, the wall heater has visible dust; and in the TODDLER 2 bathroom, a bin of diaper creams with the label to "Keep out of reach of children" and a bin containing rolls of bags were sitting on a low shelf (relocated during the visit).
The following was observed during the visit: In PREK (old PS 3), the evacuation procedures were missing from the door to the next classroom.
During the visit, the following was observed: in TODDLER 2, an educator changed two children's diapers without washing the children's hands afterwards. Educators and children must wash their hands at least at the following times: after toileting or diapering.
Playground free from hazards; fenced if applicable
The evacuation route was not posted on the door leading from INFANT 2 to INFANT 1.
The following was observed to be accessible to children: In PRESCHOOL 1, the blue wall under the "Life is a Great Adventure" hanging has a spot of peeling paint.
Two children's IHCP were incomplete. They were each missing the names of educators who were trained to administer medication and who trained them.
The fencing on either side of the playground space are approximately 35 inches in height and a body of water can be seen behind the playground space. The outdoor space cannot be used until the fencing comes into compliance with regulation.
The changing pad on the changing table in the bathroom next to PRESCHOOL 2 was ripped at the corner.
Three staff records were reviewed and found: One staff record was missing evidence of one MMR vaccine, completion of EEC Essentials 2.0 trainings, bi-monthly observations, and their PQ Registration expired 1/16/2024; and two staff records were missing bi-monthly observations (both began working in 10/2024).

Changing surface smooth and intact
No peeling or flaking paint (indoor and out)
The following was found to be accessible to children: In PREK, the cord to the boombox was loosely hanging above the heating vent and is a strangulation hazard; In PRESCHOOL 3, the wooden dollhouse is not secure and tippable; In PRESCHOOL 2, three ceiling tiles are stained and the area rug in the kitchen area has a corner curling up which makes it a tippable hazard; and in PRESCHOOL 1, a ceiling tile near the left entrance of the classroom is stained, a ceiling tile in the bathroom is stained and the two ceiling vents on the right side of the room appear to be pushing on the ceiling tiles (one tiles has cracks).
This investigation inspection recorded no violations or advisories.
Medication
Individual Health Care Plan (IHCP) - R Apr 2025
The following was observed to be accessible to children: -In TODDLER 1 and PRESCHOOL 3 the play kitchen sets were not secure and tippable. -In PRESCHOOL 3 the microwave was not secure on top of the shelf it was sitting. -In INFANT, the narrow window adjacent to the air conditioner is cracked on the inside window pane.
Physical Space Paint
The attendance in TODDLER 3 was not accurate during the visit. Nine children were present and eight children were checked in.
Required Posting
Four staff records were reviewed and found: -One staff record did not have a current PQ Registration. It expired 11/18/2023. -One staff record did not have a current physical.
The allergy posting in the INFANT/TODDLER Classroom was not covered and the information was visible to all.

Paint was observed to be chipping in the following areas: -In TODDLER 2, paint is chipping off of the windowsill below the air conditioner and on the blue wall near the bulletin board. -In the FOYER outside of the double doors on either side of the opening, small holes are visible on the walls. -In TODDLER 1, two holes are in the wall and chipping to the right of the lateral bookshelf. -Outside of the bathroom adjacent to TODDLER 1, on the wall to the right of the door, a hole was visible.
All emergency medications were observed and found: -Two children's inhalers was not in the original packaging. -One child's (REDACTED)were not in the original packaging.
The following was found: -One child's record was missing an IHCP and asthma action plan.

One Individual Health Care Plan was not signed and dated by the parent/guardian. Additionally, there was not a current medication consent signed by parent/guardian.
In proposed new Toddler Room: diaper changing table not secured, peeled contact paper accessible to children, chipped green paint on wall accessible to children. In the PreK Room, a section of the baseboard heater was broken off. In Preschool Room the bathroom was dirty, and the base of the toilet screws were missing, resulting in protruding screws accessible to children.

Infant Room staff stated that water is taken from the diapering sink to warm bottles. The same sink must not be used for food prep, and diapering.
A storage room door in the Preschool Room was not locked, hazards were accessible to children in this room. In the hallway, a door marked "employees only" was not locked; hazards in this room included a container of bleach. Another door in the hallway was open and ajar, this appeared to be a storage room; hazards included a tv on the floor, boards were leaning against the wall. The room between Infant and Toddler Rooms was open; teacher purses were accessible to children in this area. The program must ensure that these storage room doors have locks, and the areas are not accessible to children.
This renewal - monitoring visit inspection recorded no violations or advisories.
Toxic Substances
Indoor Space
Carpet in Toddler Room was torn and taped; frayed and accessible to children. Vinyl floor trim in Pre K was loose from the wall; paper on shelving units was torn. Two ceiling tiles in Preschool 2 were stained; the window sill was dirty; two areas of plaster on the walls was accessible to children. Vinyl floor trim in Toddler 1 was loose/missing from the wall. One basketball hoop was not secured. Vinyl floor trim in Preschool was loose/missing; a toilet bowl plunger was in the bathroom accessible to children, the sink stools were dirty and need replacement. Tiles at the floor drain were cracked/broken and protruding.
Adult scissors and plastic bags were accessible to children in the PreK Room. Additionally, in this room a folded up tent, and tent poles, and string/yarn was accessible to children.
