A Children's Records Checklist was not available during the visit.
The Staff Schedule was posted but one of the staff members was missing therefore it is not accurate.
The Evacuation Drill log was reviewed and since the last EEC visit in November, one drill was missed in January 2026.

The program did not have evidence of staff meetings on site during the visit. Site Coordinator stated meetings are held with the Jackson School staff.
One staff member on site (hired 1/2025) is not certified in First Aid. All educators must obtain within six months of employment, and must maintain thereafter current certification of training in basic first aid appropriate to the population served.
The children’s emergency list did not have any children listed and at least one child does have a diagnosed chronic medical condition (found during child record review).
The emergency numbers including the name and address of the program, call 911, poison control number, and health care consultant was not posted.
The first aid kit was assessed and found a CPR mouthguard is missing.
The refrigerator did not have a thermometer.
The current snack menu was not available at the time of the visit.

Three staff records were reviewed and found that one staff record did not have evidence of an interview, one staff record was not available besides a copy of their EEC Essentials 2.0 certificate, and bi-monthly observations were missing for two staff.
One staff member working at the program during the time of the visit had an expired BRC.
The current Staff Schedule was not posted in the program space during the time of the visit.
The Building and Fire Inspections are not current. The Building Inspection expired 9/1/2025 and the Fire Inspection expired 8/5/2025.
The list of emergency medical needs was not posted in the program space during the time of the visit.
One child's IHCP was not complete. It was missing the names of the educators trained to administer medication.
During the visit five children were observed to get and/or begin eating snack prior to washing their hands. The licensee must ensure that educators and children wash their hands with liquid soap and running water, using friction, in accordance with DPH guidelines. Hands must be dried with individual or disposable towels or automatic hand blow-dryers. The use of common towels is prohibited. Educators and children must wash their hands at least at the following times: before and after eating or handling food.
A medication consent form was missing for a child with an emergency medication.

The following was observed during the visit to be accessible to children: two electrical outlets in the gym did not have covers and wires were exposed. All electrical cords must be arranged so they are not a hazard to children. Electrical cords must not be frayed or damaged.
