During a review of IHCP's, it was noted one was recently expired along with the medication consent form.
This enhanced monitoring visit inspection recorded no violations or advisories.
The open field space that is fenced in and grassy did not have shade covering on the day of the visit.
The bathroom doors has locks on them, and at the time of the visit, there was not a mechanism in place to make these lock inaccessible to the preschool children who will be using the facilities
Not all of the equipment in both classrooms was secured on the day of the visit.

The program’s refrigerator which the [REDACTED] reported is sometimes used for children’s food did not have a thermometer in it, therefore there was no way to determine if food being stored in done so in a safe manner.
Through the review of the [REDACTED] Educator's staff file, it was noted that bi-monthly observations were not being consistently completed.
Individual Health Care Plan (IHCP) - R Apr 2025

The Emergency Contingency Plan as well as the Mandated Reporting of Abuse and Neglect policies did not have all of the required information as needed per regulations.
The program's uploaded Health Care Policy did not have all of the regulatory required information. The [REDACTED] was provided a technical assistance document to ensure that the missing components were addressed and the program's health care consultant approved the final policy.
At the time of the visit, there was some overgrown brush on the walkway area and around the fencing area of the proposed outdoor grassy play space.
The program did not have a complete evidence of Lead inspection or evidence that showed the building was built prior to 1987. A permit indicating a permit was pulled for "a new addition" in 1993, was uploaded, but it was not clear if this was for the original building permit for the area of where the program will be located
One gate to the fencing that surrounded the proposed outdoor play space had a gap larger than 3 1/2 inches which posed an entrapment hazard.

Although the Educators were knowledgeable about the chronic health needs of children enrolled however, the Individual Health Care Plans for the children's enrolled with such did not have all of the required information. 1) One IHCP did not have the information of whom the doctor authorized to train the staff at the program about a child's chronic health need, and which staff are trained in such. 2) One IHCP did not have information about when the program should administer one of the two medications listed on a child's plan. 3) One plan did not have information about the "potential consequences" of what would occur if the plan was not followed 4) For children who had medication(s) associated with their chronic health needs, the program did not have medications consents on file per each medication.

There was no evidence that the [REDACTED] Educator had a current BRC, as [REDACTED] most current completed BRC expired in October of 2019
Through a review of three children’s records the following was noted: 1) One child's 1st aid/emergency medical consent, off site consent and development history did not have a current date as it seemed that the parent printed the child's date of birth year rather than the date they actually signed the forms.
Peeling paint was seen on the exterior of the building to the left side of the program's entrance.
At the time of the visit, there was garden hose laying on the ground near the “bike/walking” path that goes around the playground. This placement of the hose poses a potential tripping hazard to children.
