During the visit, two staff files were reviewed. One file did not include documentation of the 12 EEC Essentials training.
During the visit, the landscape fabric was observed protruding through the mulch in several areas of the playground. This creates a tripping hazard to the children. Please replenish the mulch under the swings.
One child's epi-pen medication was not stored in the original box with the prescription label affixed.
One child with emergency medication on the premises did not have an Individual Health Care Plan or Medication Consent Form.
Staff Records
A review of staff records indicated the following: -One did not have documentation of a current physical examination. -One did not have documentation of completing the EEC Strong Start Essentials -One did not have documentation of successfully completing a First Aid Course.
This monitoring visit inspection recorded no violations or advisories.

The licensee did not have hand washing and toileting plans posted in the bathroom. A written plan for toileting must be posted in toileting areas.
Plan for diapering and toilet training
The licensee did not have a current building inspection certificate. Every program that is not located in a residence and every program that has a licensed capacity of more than ten children must provide a certificate of inspection from the local building inspector.

A review of three staff records indicated the following: -Three did not have evidence of bimonthly supervision for the month of January 2020. Observation of educators while working with children at least every two months by a staff with lead teacher, site coordinator or higher qualifications.
The building inspection expired on 6/1/2019. Please upload a current building inspection to the LEAD Portal. Every program that is not located in a residence and every program that has a licensed capacity of more than ten children must provide a certificate of inspection from the Department of Public Safety or the local building inspector certifying that the facility complies with the applicable 780 CMR: The State Building Code.
