At the time of the visit, the Preschool 1 classroom was combined with the PreK classroom. Emergency medications for two Preschool 1 children did not transition with them to the PreK Classroom. Therefore, the emergency medication was not immediately available if needed.

During the visit, three staff files were reviewed. One file did not include verification of 2 references. One file did not include all of the required documentation to show the staff member was teacher qualified. One file did not include evidence of immunity to Measles, Mumps, and Rubella. One file did not include evidence of Program Orientation.
The program did not have an Individual Health Care Plan for a child with an inhaler on the premises.
In the Toddler classroom, two feeding spoons were observed in the handwashing sink. Sinks used for hand washing after diapering or toileting must be separate from sinks and areas used for food preparation and food service.

During the visit, three children's files were reviewed. Two files did not include a current First Aid Medical Consent Form. Two files did not include a current Transportation Plan. Two files did not include a current Developmental History Form. One file did not include a current physical.
The staff schedule posted was not current.
The program's building inspection and fire inspection expired on 8/23/2023.
There was not an adequate depth of impact absorbing material under and around the fall zones of the climbing structures on the playground.

The toilet seat in the Preschool bathroom was chipping.
A review of three Individual Health Care Plans (IHCP) indicated that three plans were incomplete and did not include which educators at the program were trained to address the medical conditions.
In the Preschool classroom the children's sleeping materials were stored in large plastic garbage bags. The bags were organized in a corner, however, were accessible to children.
A review of the staff meeting documentation indicated that the times of the meetings were not recorded.

The building inspection certificate at the program expired on 4/1/2022. Please submit a current certification.
An(REDACTED), for a child attending the program, was being stored in their personal backpack and was being transported between home and the program. The licensee must maintain an on-site medication for use as needed. The (REDACTED) was also located within reach of a group of toddler aged children. Unless otherwise specified in a child’s individual health care plan, the educator must store all medications out of the reach of children.

The last emergency evacuation drill was documented on 2/11/2020. The provider must hold practice evacuation drills with all groups of children and all educators from each floor level of the approved space at least monthly.
