Staff Records
During a review of three Children's Records at the time of the visit, there were six medications prescribed for diagnosed medical conditions. Out of these six medications, the following was identified: -One prescribed medication for a child was not stored onsite. The Director stated during the exit interview on 3/25/25 that the child does not require the medication at the program according to the parent. Technical Assistance was provided to have this information in writing by the child's physician and/or parent. -One medication had expired on 12/2024. -Two medications were not contained in the original dispensed box with affixed prescription label.

A review of three Staff Records at the time of the visit indicated the following: -Two staff records did not contain evidence of a current physical examination. -One staff record did not contain evidence of immunity to MMR.
During a review of three Children's Records at the time of the visit, the following was identified: -One record did not contain evidence of an Individual Health Care Plan for a child with a diagnosed medical condition. The Director stated during the exit interview on 3/25/25 that they did not think they needed an IHCP for this as the child does not require medication while in the care of the program. Technical Assistance was provided by Licensor on 3/25/25.
A review of three Children's records and six medications indicated the following at the time of the visit: -There was not a Medication Consent Form for two out of six medications stored onsite. Each medication must have a Medication Consent Form authorized by the parent/guardian of the child allowing the program to administer the child their prescribed medication in the event of a potential medical emergency.
This enhanced monitoring visit inspection recorded no violations or advisories.

Upon a review of three Children's Records during the visit, the following was identified: -Three Children's Records did not have complete and current Developmental History and Background Information Forms or First Aid and Emergency Medical Care Consent Forms.
There was an inadequate depth surface of wood chip nuggets and sand on the playground around the swings and structures. During the visit, the Director stated that the program will replenish these materials during the summer.

Toxic Substances
There was a tan sandbox in the use zone of one tall composite structure.
A sharp knife was in the dish drainer in a sink in the downstairs preschool room. This was accessible to children.
One child was an (REDACTED). The (REDACTED) at the program expired in (REDACTED) and the (REDACTED) was not in an original box with a (REDACTED) label attached.
