The program's building inspection certificate expired 2/6/25. The fire inspection also expired on the same day. During the visit, the Director learned from a facility staff member in the public school that the town is scheduled to come to the program on 4/22/25 for an building and fire inspection certifying that the facility complies with the State Building Code (780 CMR) and applicable fire codes.

The program does not have a current Building and Fire Inspection Certificate. During the visit, the Licensee stated they are working with the town and public school to have the town inspectors conduct their annual visit.

A review of three Children's Records at the program indicated the following: -Three individual children's records did not include a written annual progress report.
A written progress report must be done periodically
The program did not have evidence of emergency evacuation/fire drill logs for the months of January and February.

A walk through of the facility found the following: -There is chipped/peeling paint throughout the blue colored walls of the gym. This is most notable to the left of the main entry/exit door and to the left of the duct smoke detector near a second exit door.
A review of medication stored at the program found the following: -One child's prescribed (REDACTED)expired (REDACTED). -One child's prescribed (REDACTED) expired (REDACTED). -One child's prescribed(REDACTED) was not in its original container with affixed prescription label.
This investigation inspection recorded no violations or advisories.
The Building Inspection Certificate expired 5/27/21. The Fire Inspection was completed on 10/2/20.

Upon review of three children's records, three lacked a complete Individual Health Care Plan (IHCP). The name of the educator that received training in the administration of medication and the person that trained the educator need to be included in the IHCP.
