The circle time rug was worn down and ripping. The floor under and around the trash cans have cardboard under them, the cardboard is torn, dirty and collecting dust and dirt. The carpet under the art table is torn, not secured down to the ground. The red carpet in the home living area is rippled and not secured down causing a potential tripping hazard. The blue Lego bin had chipping paint. The TV in the preschool room was not secured down.

Documentation of medication administration; Current consent for medications
The children's sleep mats were torn exposing the stuffing, some mats where duck taped in several areas.
The stool at the sink is collapsible and tippable.
There was no documentation of fire drills from June to November 2024. It was reported monthly fire drills have not been conducted in the past.

The individual Healthcare Plan for a child with allergies does not specify when to administer the medications prescribed. The educators were not knowledgeable about what medications to administer and when.

Staff records did not have current bi-monthly observations or annual evaluations. The Program Director reported bi-monthly observations and annual evaluations are not being completed.
During the visit children arrived at 11:15 and they were not signed in, until 11:48 when the licensor asked why three children in care were not on the attendance sheet at 11:48.

Children’s records had expired first aid consent and transportation plans. One child had an expired physical and needed a lead test.

Medication
A little tikes climbing structure was missing the slide. The structure’s safety is compromised without the slide. The climbing structures do not have a 6 foot fall zone, they are to close to other structures and the fence.
This renewal - full review visit inspection recorded no violations or advisories.
During the review of Staff, the following items were non-compliant: 1)There was not evidence that one staff member had a file. 2) The staff working in the school age program did not have verification of the requirements of their position, including work experience and educational attainment. 3) A staff was missing evidence that observations, by a Lead Teacher qualified person or above, are conducted while staff are working with children, every other month. 4) A staff member was missing a physical. 5) Two staff members were missing the following: a) application, b) physicals, c) evidence of immunity to MMR, d)Evidence of a current EEC Professional Qualification Registry. e) Evidence of completion of the required Essential Training.

One child’s file had First Aid consent, transportation plan, physical, IHCP and medication consent forms expired. One child file did not have an IHCP for intermittent ASTHMA that was noted on their most recent physical.
A blue children's couch in the school age area was peeling and non washable. The two wooden sensory table tops, and the children's play kitchen set and sink were worn down discolored and porous, preventing them from being properly sanitized and disinfected when needed. Due to the condition of the equipment the children could potentially get a sliver or pick up germs that got trapped in the wood.
The green rug, red rug and brown rug were stained and taped to the floor, the tape was peeling and the adhesive was exposed. The red rug was torn and had runs in it. The classroom had several outdoor plants that lined the windows within reach of children. The plants were not labeled and the licensor could not identify if they could potentially be poisonous or dangerous to children.
There was an expired consent form for one child with an inhaler. Another child was missing a medication consent form for an inhaler.
The painted blue shelf, yellow shelf and the painted lego bin had peeling chipping paint throughout the whole unit.
Building inspection expired October 2024.
Children's shelves were covered with cloth and materials were not visible or accessible to the children during the visit.
One epi pen was expired.

On the day of the visit, there was not a completed current Staff Records checklist.
On the day of the visit, there was visible clutter in the teacher area that is connected to the school age classroom. In addition, the closet that leads to the required second bathroom had excess clutter.
On the day of the visit, the posted Staff Schedule was not current.
On the day of the visit, the licensor observed that the program brought children to the playground, that is not attached to the building, and did not take a child's emergency medication with them. Emergency medication must be easily and readily available at all times and accompany the children anytime they leave the facility in the care of staff.
On the day of the visit, two Individual Health Care Plans, (IHCP) were reviewed and did not contain all the written required information.
On the day of the visit, the Building and Fire inspections were not current.
