Outdoor Space
It was observed that there was a hazard in the outdoor space. The wooden structure with the swing and slide was not anchored below the surface, there was no protective surface in the fall zones under and around the equipment, and the structure did not contain signage or manufacturer's information to indicate age use or that it was commercial equipment.
This pre-licensing visit inspection recorded no violations or advisories.
