During the visit, the records of the 3 children present were reviewed. The following was observed: -CHILD #1: lead screening results were not up to date in the file. -CHILD #2: doctor's information was incomplete in the enrollment packet. -CHILD #3: developmental history was incomplete in the enrollment packet.

During the visit, 4 children emergency cards were incomplete and 2 emergency cards were not up to date in the first aid kit.
During the visit, educator stated that household Member/Person {REDACTED} moved back to the home at the end of September 2025. The educator failed to notify EEC within 7 days of this change in the household.
EEC regulations states that the changing surface is protected with a covering that is of adequate size to prevent the child from coming in contact with the changing surface. During the visit, educator used the diapering plastic mat on the Lower level and changed a child's diaper, however no covering of adequate size was observed between the child and the diapering/changing surface.
During the visit, the following was observed: 1) educator stated that a household member/Person {REDACTED} moved back to the home at the end of September 2025. Person {REDACTED} does not have a background record check free of conduct which, in the judgment of the Office, bears adversely upon applicant's or licensee's ability to care for children. 2) educator has a household member/Person {REDACTED} living in the home who does not have an up to date background record check.

During the visit, educator did not have a list posted in a confidential manner, including a child with allergy to food/milk.
Last drill document for the Lower level was December 2025. After reviewing the evacuation log for the year 2025, the following was observed: 1) EEC regulations states that the educator must hold practice evacuation drills with all groups of children and all educators from EACH floor level of the approved space at least monthly. Educator has the 1st floor and Lower levels licensed at this time. The evacuation log showed that practice drills were only completed from January to December 2025 from the Lower level. No practice drills were complete from the 1st floor level as required. 2) The evacuation log did not showed how long practice drills took to complete from January to December 2025. 3) EEC regulations states that drills must be held during different times of the program day, and must use alternative exits. At this time educator has 2 separate exits from the Lower level and 2 separate exits from the 1st floor level. Educator has 2 exits from the 1st floor level. The evacuation log did not showed that the exits located on the 1st floor level were used to practice drills from January to December 2025.

During the visit, educator did not have thermometers inside the refrigerators located in the 1st floor and Lower level to verify refrigeration and storage for food at not less than 32 F, nor more than 40 F degrees.

EEC regulations states that educators and children wash their hands with liquid soap and running water, using friction, in accordance with Department of Public Health guidelines after toileting or diapering. During the visit, the educator changed a child's diaper on the Lower level. The educator washed her hands after changing the diaper; however, the child's hands were not washed as required.
During the visit, 2 separate fireplaces were observed in the first floor level. Per educator they are not used for heating purposes. However, the hearths of the fireplace located in the living room area was not protected or padded.
EEC regulations states that diapering surfaces must be washed and disinfected after each use. During the course of the visit, the educator was observed changing a child's diaper on the Lower level . However, the diapering surface ( white plastic mat) was not cleaned and disinfected after it use as required.
During the visit, the following was observed: 1) ECC policy states that all exits must be clearly marked with visible exit signs posted above the approved exit. During the visit, educator did not have exit signs for the exits located in the 1st floor level. 2) The route of the back exit through the lower level and leading to the backyard was obstructed with snow and was slippery. 3) The back exit from the 1st floor level leading to above ground level deck, the stairways were slippery due to the snow. Additionally, an uncovered grill and a propane gas tank were observed accessible to the children in the deck area along the exit route.
Educator had 4 plastic mats and 3 playpens. However, blankets were stored inside a closet in the Lower level and pile on each other. The blankets were not store in a safe and sanitary manner.

The educator has only CPR and First training online expired on 2/26/2024. Licensor explained to the educator that CPR and First Aid training online are no longer acceptalbe. Licensor informed the educator to take CPR and First Aid Training with the Trainer.
