Staff personal belongings (Purse) were stored inside an unlocked closet in two classrooms, accessible to children in the room.
1. Documentation of an updated physical examination was not available for review in a staff's record file. 2. Documentation of a program orientation was not available for review in a staff's record file.
A child’s prescription medication (EPI Pen) was not stored in the original packaging/box in which medication was dispensed.
Some peeling of paint was observed from a windowsill and a classroom door.

A child’s Individual Health Care Plan dating 7/24/2025 did not reflect written parental consent.
1. Documentation of a current transportation plan was not available for review in two children’s record files. 2. Documentation of a current developmental history form was not available for review in a child’s record file. 3. Documentation of a current lead screening was not available for review in a child’s record file.
Physical Space Paint
A medication consent form for an over-counter medication (Tylenol) did not reflect written parental consent.
Several personal items intended for individual use by children (sippy cups/water bottles) were not labeled with the name of the child for whom they are intended.
This investigation inspection recorded no violations or advisories.
A child’s emergency medication (Epi Pen) indicated expiration date of July 2024.
Documentation of written parental consent was not included in two children’s individual health care plans, diagnosed with a chronic medical condition.
1. At the time of the visit, an air freshener indicating “Keep Out of Children” was stored inside a toy cart located in the indoor gross motor room. 2. At the time of the visit, a staff’s personal belonging (tote bag) was placed on top of a classroom shelf, within reach of children.
A child’s hands were not washed with soap and running water following a diaper change.

During the visit, several classrooms were observed to have evidence of chipping, flaking, or peeling paint.
In one classroom, an educator inaccurately reported the number of children present in the classroom; in another classroom, the written attendance was inaccurate (one child had left and was not signed out).
At the time of the visit, 3 staff members did not have evidence of current physicals.

One of the reviewed {REDACTED} IHCP documents did not record a parent signature.
EEC observed the following potential hazards, at the time of the visit: The lower cabinets located in the {REDACTED} classroom were not locked and contained scissors and soap. A vinyl couch intended for child use was fraying / ripped and located in the {REDACTED} classroom. Items were stacked too high on a rolling storage unit in the {REDACTED} classroom.
The bathrooms located in the {REDACTED} classroom were unclean. Several balled up paper towels / toilet paper pieces were strewn about the floor at the time of EEC's visit.

An {REDACTED} child was practicing tummy time on a Boppy Pillow without an {REDACTED} staff member within arms reach. EEC reviewed safe tummy time practices with {REDACTED} staff and the program's {REDACTED}.
Adequate depth of surfacing was not evident at the end of the slide structure and / or raised structure. Please fluff / rake the mulch and / or add mulch to the playground's floor.
