The vinyl corner piece on the bottom right side of the shed is broken with sharp edges exposed.

During the visit, four staff files were reviewed. Two files did not include a current physical. One file did not include evidence of immunity to Measles, Mumps and Rubella.
The pour-in-place rubber surfacing under and around the climbing structures in the preschool and toddler playgrounds is no longer flush with the paved walkways and poses a tripping hazard for the children.

A toilet plunger was accessible to the children in the preschool bathroom. A wooden kitchen cabinet was not secured in the Toddler 2 classroom.
Weeds were observed growing around the preschool climbing structure and unknown vegetation was growing through the chain link fence.
There was one stained ceiling tile in the Preschool 2 classroom. There was tape residue on the rear door in the Infant 2 classroom. In the Infant 2 classroom, the fabric was torn on two bouncy chairs and not in good working order.

The following hazards were observed on the playground: the vinyl siding was broken in several places on two storage sheds, leaving sharp edges accessible to the children. There is unknown vegetation growing through the chain link fence.
One child's Individual Health Care Plan did not include all of the required information.
