The following was observed to be accessible to children during the visit: In TODDLER 2, tape residue was visible on the surface of the wooden shelf with musical instruments, the floor had crumbs and dust next to the left side of the wooden shelf with construction vehicles; and in the gym area, there was a hole in the gray wall with plaster showing near the electrical outlet. The following incident (on 10/17/2025) resulted in an (REDACTED) when the educators failed to monitor the gym area to identify and remove or repair any hazards that may cause injury to children. The child (REDACTED) that had likely been tracked in on someone's shoe (case 00164209).
During the visit, the emergency posting in the Toddler 2 classroom displayed two children's names not in a confidential manner. The two names were listed on the front of the posting where visitors and parents could see. Protected health information must be posted in a confidential manner.
The emergency medications were reviewed during this visit and were present, however, a measuring cup was missing for a child's antihistamine (used as an emergency allergy medication).

Seven child records were reviewed and found: one child's CPR/First Aid consent and transportation plan were not current (expired 9/10/2025 and 8/26/2025 respectively).
Three staff records were reviewed and found: One staff record is missing evidence of immunity to MMR and an incomplete orientation checklist. Another staff record is missing evidence of one reference check.
The new playground installed in April 2025 was observed for the first time during this visit and it was found the manufacturer's sticker adhered the the equipment stated it is intended for residential use and ages 3-12. The Program Director stated toddler and preschool age children are allowed to use the equipment with adult supervision. During the visit, preschool age children were observed to be outside using the equipment and two toddlers groupings had been outside earlier in the visit. The licensee must only use indoor and outdoor equipment, materials, furnishings, toys, and games that are appropriate to the ages, needs and developmental level of the children enrolled. They must be sturdy, safely constructed and installed, non-tippable, flame retardant, easily cleaned, and free from lead paint, protruding nails, rust, and other hazards that may be dangerous to children.
The program installed a new playground in April 2025 and have allowed children to use it since installation without pre-approval by the Department. The licensee must notify the Department when possible, at least 30 days prior to any change in the space used by the program. If the licensee is unable to provide 30 days notice, the licensee must notify the Department by telephone immediately upon learning of the impending change.
The following was observed during the visit in the outdoor space: the far side gate is closed but is no longer closed straight therefore there is a large gap between the gate and the fence post. This space is wide enough for a small child to squeeze through and exit the outdoor space.

The Building Inspection is current (expires 3/7/2024) but the Fire Inspection has expired (5/12/2022). The Director stated the Fire Inspection is scheduled to be done in November.

The following was observed to be accessible to children: -In TODDLER 2, tape residue was in multiple spots on the long white table and on top of the bookshelf next to the circle rug. -In the LIBRARY, five ceiling tiles had stains indicating they had been wet. -In PREK (PS/SA), the lateral bookshelf below the white board was not secure and tippable and the trash can did not have a cover.
Two Individual Health Care Plans were incomplete. One child's Individual Health Care Plan was missing the side effects of treatment, the consequences of not receiving the treatment and who trained staff to administer. Another child's Individual Health Care Plan was missing the side effects of treatment.
