Cited finding

The licensor reviewed 3 of the children records. The following were observed: 1. Two of the children records did not have a picture or a physical description. 2.Three of the children records did not list the assistant name as part of the general permission (page 6 of the enrollment). 3.Three of the children records did not on the general permission where children can go where off the premises (it needs to specify). 4. One of the children record did not have an updated physical (last physical was 7/23/23). 5.One of the children record is incomplete and should review pages 5 and 6 and complete the empty spaces).
