The licensor made the following observations: The 2 preschool bathrooms have accessible plungers; the Toddler classroom closet containing a staff handbag is accessible to children.
The licensor reviewed 2 staff files with the following findings: 1/2 staff do not have evidence of immunity to measles and mumps; 1/2 staff member's last bi-monthly observation was 4/2025.

Outdoor Space
The licensor reviewed 3 children's files with the following findings: 2/3 children do not have lead test results on file.
Licensor walked through the outdoor space and made the following observations: a yellow extension cord handing from the roof is accessible to children; black rubber mats placed in play structure fall zones are a tripping hazard; the roof of a small storage shed is broken and accessible to children. The program director stated that a play structure recently delivered to the program will not be used until it has been secured. The program director stated that the wood chips will be replenished in the spring and will include all fall zones for the existing structures.

There was a toilet plunger accessible to children in the hallway bathroom.

Emergency medications readily available
At the time of the the visit there was not adequate shock absorbent materials under the fall zones of the swings and climber.
At the time of the visit three children with emergency medication were reviewed and the following was found: two children were missing one of their two prescribed emergency medications.

Portions of the staff records checklist were not current. Please update and upload a new version to the document library using the newest version. At the time of the visit, three staff files were reviewed and the following was found: one file did not contain evidence of reference being conducted.

2 children have EEC IHCP forms on file that have not been signed by a parent; EEC IHCPs forms for 2 children are not complete. 2 children have an EEC Medication Consent form that is not complete; 1 child has an EEC Medication Consent form on file that has not been signed by a parent.
At the time of the visit one medication consent form was missing a parent signature and one medication consent form was missing for a child's med.
Classrooms did not have soap/water bottles readily available at the time of the visit.
