Mulch in outdoor space needs to be replenished and must always have 9 inches of depth for fall zones for the climbing structures.

Smoke detectors could not be tested at time of visit due to sleeping child.

Attendance for today’s visit was not documented.
Educator has three climbing structures, (two slides and teeter totter) that did not have the required 6 foot all around fall zones.
Educator did not have proof of 30 hours of required ongoing professional development training hours at time of visit. Educator will need to submit proof of ongoing professional development trainings.
Posting for children with allergies/medications were not posted.
A random review of children’s files was completed. It was observed two files did not have an individualized health care plan/allergy action plan for those children that have allergies and/or require medication. Files were not up to date; annual updates are due for all children. Educator did not have parent permission form to administer medications.
Based on review of attendance logs, educator has not been keeping accurate attendance logs for the past year.
Educator had a CPR and First Aid certification that expired on 12/2021. Educator indicated that she has a more recent certification however proof of these certifications could not be provided during the visit. Educator indicated that she will attempt to complete a CPR and First Aid certification class by today. It was discussed with the educator that is she is unable to submit proof of up to date CPR and First Aid she is required to close her program until she is able to submit proof of current CPR and First Aid.

It was observed that door was closed to the room where child was napping. Discussed with the educator when children nap in separate rooms, door mus always be ajar. Educator opened door.

Children were observed washing hands and using same cloth towel to dry hands. Each child needs to have an individual towel.

Review of children’s files was completed. Files did not have the most recent progress reports on file.
Evacuation log was reviewed; it was observed that evacuation drills from March 2022-May 2022 were not documented. Licensor had the educator conduct an evacuation drill during this visit.
Outdoor climbing structures did not have the required 6 foot all around fall zones.
