A review of two staff records indicated that both did not have evidence of a current physical examination, one had an expired EEC PQ registration, one had an incomplete program orientation of hours completed, and no evidence of current bi-monthly observations.
The following were observed and accessible to children: There were rips in the green couch and three uncovered outlets.
The fire inspection was expired. The Executive Director stated that a visit is scheduled 6/6.

When the Licensor arrived to the program, the staff were initially unable to identify the qualified person designated in charge on site. Once the Site Coordinator was identified, they stated that they did not have access to all of the licensing documents.
