It was observed in TODDLER 1 that the garbage container was not covered.
It was observed throughout the program that evacuation diagrams were faded and no longer showed escape routes.
It was observed in the TOODLER 1 classroom, that there were 2 water bottles that did were not labeled with the child's name.
The following was observed and accessible to children: 1) In TODDLER 2, there was chipped paint on the wall between the front windows. 2) On the PRESCHOOL OUTDOOR PLAY AREA, there was chipped and flaking paint on the four seated seesaw.
The program was inspected on 3/13/26, but a certificate has not been issued.

The following was observed and accessible to children: On the TODDLER playground, the back right corner section of fencing was loose and a potential entrapment hazard, the plastic play kitchen and basketball hoop were not secured and tippable. On the PRESCHOOL playground, the plastic play kitchen was not secured and tippable with sharp edges, the red ride-on truck had broken windows with sharp edges, and the four-seat seesaw had a broken seat that was cracked and partly uncovered, and a small section of chipped paint (the seesaw was previously cited on 9/21/21, 3/21/22 and 1/27/23).

The following was observed and accessible to children: -The plastic tool bench had a large crack that was previously covered but no longer was. -The sandbox had multiple cracks along the border. (This was previously cited on 1/26/22).
It was observed that a climbing structure 30 inches high did not have a fall zone of 6 feet around the perimeter. It was located next to the side fence.
Fire Inspection recently expired within three days. Fire Department is expected to come in March.
A review of the fire drill log indicated that there were two drills that did not have the number of children documented.

It was observed that two Educators were changing a child while another child appeared upset and cried for approximately five minutes before an Educator was able to tend to them. Educators must not engage in any other activities or tasks that could unnecessarily divert their attention from supervising the children.
It was observed that after an infant's soiled diaper was changed, two Educators and the infant's hands were not washed and dried.
It was observed that a changing table was not washed and disinfected after a child's diaper was changed.

The following was observed and accessible to children: PRESCHOOL 2 - There was a pair of sharp adult scissors and a stapler located in a bin on top of the closed sensory table. PRESCHOOL 3 - There was a pair of sharp adult scissors on top of the play kitchen. PRESCHOOL 4 - There was an open box of vinyl gloves on top of the play kitchen and there were plastic bags located in the unlocked cabinet under the classroom sink. These are potential choking hazards. (This was previously cited on 3/23/2022, 9/21/2021, 2/18/2020, 9/5/2017).
The following was observed and accessible to children: There was a cracked plastic toy storage container. (This was previously cited on 3/11/2022).
The following were observed and accessible to children: TODDLER 1 and TODDLER 2 had unsecured electrical cords; TODDLER 1 - A windowsill molding was coming off the wall and the Licensor was able to place fingers underneath. This is a potential entrapment hazard; There were cracked floor tiles in the TODDLER 2 BATHROOM and PRESCHOOL 1 classroom; There were unsecured and tippable furniture in PRESCHOOL 2, PRESCHOOL 3 AND PRESCHOOL 4 (The Toddler 2 floor tiles and unsecured furniture were previously cited on 9/21/2021).

It was observed that there was chipped and peeled paint throughout the indoor and outdoor space of the program. A review of the program's lead paint inspection indicated that they are in compliance due to being built after 1978. (This was previously cited on 9/21/2021).

The following were observed and accessible to children: FRONT PORCH - There were two broken wooden rocking chairs missing panels on the seat, an uncovered electrical outlet and a cracked siding panel. PLAYGROUND - There were pieces of trash and plastic bags located on both the Toddler and Preschool playgrounds; the chain link fence that separates the Toddler and Preschool playgrounds had an unsecured bottom edge that is a potential entrapment hazard; a cracked plastic toy storage container, the bike-go-round had a missing pedal on one bike and another bike was off the track; the four-seat seesaw was rusty with a broken and cracked plastic seat and another seat was completely missing; three plastic play houses had cracks with sharp edges; the sandbox had sitting water; the large climbing structure had chipped paint; the play kitchen was unsecured and tippable. BRICK WALKWAY - There was a raised border edge that is a potential tripping hazard. GRASS AREA - Behind the shed, there was sharp landscaping equipment. (The bike-go-round, four-seat seesaw and shed area were previously cited on 9/21/2021).

A review of the Evacuation Drill log indicated that the route taken was not indicated for the drills conducted in June, July and September 2019.

A review of medication administration documentation for six children with and without a chronic diagnosed condition indicated the following: three were not fully completed to include side effects, staff trained, and who trained the staff. One form was expired. Another form was missing a parent signature.
During the visit, it was observed on the FRONT DECK that one riding toy had tears on vinyl seat and one had a ripped foam handlebar cover.
During the visit, the following was observed and accessible to children: The trash container in TODDLER 1 was uncovered and contained discards food items and there was a cracked outlet plate, there were cracked floor tiles in PRESCHOOL 2 and PRESCHOOL 3, there was an unsecured extension cord located in PRESCHOOL 2 and unsecured fish tank light and filters cords in PRESCHOOL 3, and the lightbulb in the PRESCHOOL 3 BATHROOM needs to be replaced.
During the visit, it was observed in TODDLER 1 that there was an open staff purse located on a low ledge in the corner accessible to children.
A review of staff CPR/First Aid training indicated that one Educator did not have evidence of a current first aid certification.

Cleaning, Sanitizing, and Disinfecting Procedures
It was observed that an infant was in their bassinet with a pacifier attached to their clothing by a beaded pacifier clip. This is potential cause for choking or strangulation hazard.
It was observed that there was one qualified Educator in the infant classroom with four children. An unqualified Assistant was outside with three infants.
The following were observed and accessible to children: TODDLER 1 - The white shelf in the corner was unsecured and tippable. (This was previously cited on 9/21/2021). TODDLER 2 - In the bathroom, there were cracked floor tiles. (This was previously cited on 3/23/2022). PRESCHOOL 1 - There were electrical cords hanging next to the classroom door. All electrical cords must be arranged so they are not a hazard to children. PRESCHOOL 2 - There was an unplugged electrical cord hanging from the power strip on top of the multicolored supply rack. (This was previously cited on 9/21/2021). Throughout the program it was observed that there were windows without a protective screen. Windows and glass doors must be constructed, adapted, or adjusted through the use of window guards or other means to prevent injury to children. In addition, it was observed that window blind strings longer than six inches were not kept out of children's reach.

A review of the Toddler 1 and Toddler 2 attendance records indicated that one child was not signed in. When addressed with an Educator, they were aware of who was not signed in and was immediately corrected. (This was previously cited on 9/21/2021 and 9/5/2017).
It was observed that there wasn't evidence of a curriculum plan posted or activities done in the Toddler 1 and Toddler 2 classrooms.
It was observed that in the Preschool 2 classroom, there was an open snack bag on the table. Food needs to be stored in a covered container. (This was previously cited on 9/21/2021).
It was observed that in the Preschool 2 classroom, at approximately 10:20am, children's lunchboxes were set out on the table. On the table was a cup filled with a dairy or dairy alternative. Meals and snacks provided by parents must be stored safely. According to the classroom schedule posted, lunch was not until 11:30am-12:00pm.
It was observed that the door to the kitchen area next to the main entrance was open and accessible to children. Inside the door was a push broom, electrical appliances and cleaning products. (This was previously cited on 3/21/2022, 9/21/2021 and 2/18/2020).
It was observed that an emergency and evacuation procedure was not posted in the School Age classroom at the secondary exit door leading outside. (This was previously cited on 9/21/2021 and 2/18/2020).

The Kitchen area was accessible to children through the open door in the main hallway next to the main entrance. (This was previously cited on 9/21/2021 and 2/18/2020).
In the Toddler 1 and Preschool 4 classrooms, there was no evidence of an emergency numbers posting.
It was observed that throughout the program, the location of a First Aid kit was not posted or identified.
The emergency exit door in the Preschool hallway did not have an evacuation positing. (This was previously cited on 9/21/2021).
The following were observed and accessible to children: SCHOOL AGE - In an unlocked cabinet, there was a glitter spray paint can. The product label indicated "keep out of reach of children." KITCHEN - In an unlocked cabinet underneath the sink, there were plastic bags and an open bottle of cleaning solution. PRESCHOOL 4 - On top of the low teacher cabinet, there was a bottle of correction fluid. The product label indicated "keep out of reach of children."
A review of one medication indicated was not appropriately labeled with the prescription label including the child's name affixed to the box.

The following were observed and accessible to children: TODDLER 2 - Near the windows, there was a tippable plastic work bench not secured. PRESCHOOL 3 - On the right side of the room, the number rug had a corner that was curled up; potential cause for a trip hazard.
It was observed in Preschool 1 that quiet activities were not provided to children who were awake for approximately 30 minutes.
It was observed that the garbage containers in the School Age room and Infant 1 room did not have a cover.
The following were observed and accessible to children: SCHOOL AGE - On the ceiling above the cabinets, there was a hole with flaking plaster. INFANT/TODDLER HALLWAY - Outside the Infant 2 room, there was an open ceiling space exposing a pipe. TODDLER 2 - Next to the table, there was a cracked floor tile. INFANT 2 - Near the door, there were cracked floor tiles. PRESCHOOL 3 - On the ceiling in the left corner of the room, there was a water stain.
It was observed throughout the program, an evacuation procedure was not posted at the secondary exit doors in the classrooms and at the hallway exits leading outside.
It was observed that in the Kitchen, there were two dry food items were not stored in a sealed container.
A review of four Individual Health Care Plans indicated that all were not fully completed to include which staff were trained and who were they trained by.
The following were observed and accessible to children: SCHOOL AGE - In an unlocked drawer near the Kitchen, there were two sharp knives. The Licensee removed them immediately. STORAGE SPACE - In the room connected to the School Age room, there were two bottles of cleaning solution with manufacturer instructions to "keep out of reach of children". KITCHEN - Next to the sink, there were two vacuums with cords approximately 6 feet attached. These vacuums were immediately moved to another space. INFANT/TODDLER HALLWAY - Outside the Toddler 2 room, there was a vacuum. This was also immediately moved to another area. PRESCHOOL 3 - The math rug had a curled up corner; potential cause for a trip hazard. PRESCHOOL 4 - In the unlocked sink cabinet, there were plastic bags. When discussed, the educator removed them immediately.
